We're back with the endodontic series and we're ready to bust out our rotary files.

But first..do you even play video games bro? Well Kevin used to and he had a pretty big obsession with Princess Peach. Zach learned everything he knows about football from the original Tecmo Bowl and like the Final Fantasy purist he is, he wishes you could just cast a Cure spell rather than have to do some fillings. Would you like to see Zach and Kevin play Blades of Steel against each other online? Why isn't this possible in 2020...or is it?

Kevin is using a new Woodpecker AI motor.  and is enjoying it.

Zach uses a Woodpecker 3 apex locator.

Kevin uses mostly the Edge Endo GTX System.

Zach uses the Edgetaper Platinum System.

Don't miss out on the glidepath files for small canals.

What irrigants do we use?   How about file lubes?  (yes, we said lube).

Is rotary endo too fast? How do we make sure the irrigants sit long enough?

We're getting closer and closer to finishing this root canal...check back next week when we get to obturating this tooth!

Dr. Artie Volker joins the Dental Hacks again, this time to discuss digital workflow and the lab industry! Artie has actually built a lab in his office and has a unique perspective on how dental workflows will change in the future!

  • Why did Artie build a commercial lab in his office?
  • How many people do you need for a dental lab?
  • Can you have a dental lab without stone models?
  • Why does milling zirconia in office make so much financial sense?
  • When you do your own lab work, what do you learn about your preparations?
    • make sure you have occlusal clearance
    • recontour adjacent teeth!
  • Al says have very fine finishing burs for adjusting zirconia
  • Artie is stacking porcelain (NERDS!)
  • Artie is using for Lab Pronto orthodontic aligners!
  • How do labs stay profitable moving forward?
  • Working DIRECTLY with a lab tech (e.g.: in the operatory)
  • How to start in digital dentistry according to Artie!

We continue our spinoff of a spinoff of a podcast this week and talk Saturday night live for the first 10 minutes. Specifically our favorite 3 cast members and some of their skits.

On this week's continuation of the endo series we discuss access:

  • Kevin likes long shanks, and let's be fair who doesn't.
  • Both of the guys like round burs
  • If you haven't studied any modern access techniques and you graduated in the 00's or earlier it would probably benefit you to revisit best practices for rotary or reciprocating files.
  • Both of the guys like endo explorers.
  • ZAM likes ultrasonics for troughing difficult to find canals, just like the fancy euros on YouTube.
  • Great endo youtube Channel - John Rhodes
  • When do you punt a case if you can't find the canals?
  • What do you do when things go awry?
  • What are some real batting averages for finding MB2?

Next week we finally get into filing and irrigating this tooth!

See ya!

    • Learning at your own speed/on your own time

  • Creating great relationships with a lab (Dennis primarily uses Synergy Ceramics)
  • Dentistry on social media (the good, the bad and the ugly)
  •  As an educator, what do most restoratives get "wrong" most often? (spoiler: the answer might surprise you!)

Frank opened the meeting with a talk that discussed the stories that dentists tell each other about where the profession is going and then presented some data that suggests a lot of these stories aren't exactly true. Some of the things we discussed:

  • Why do insurance companies cover thing so differently from state to state?
  • The beauty of practicing in groups. But not necessarily a DSO.
  • Could someone become Frank Spear in the way that Frank did considering student debt as it is now?
  • How did beautiful temporaries make a difference in Frank's career?
  • Why does Frank feel responsible to help the narrative that dentists tell each other?
  • What's the data the Frank would really like to see?
  • Why is it so impossible to compare dental businesses and income across the country? How can you compare apples to apples?
  • Take home message: things are better than dentists are telling themselves!
  • What is dental school like when your dad is Frank Spear?
  • How could dental school teach things with more context?
  • Are there useful parallels between dental and medical education?
  • How will Frank's teaching schedule be changing in the future?
  • How is the curriculum at Spear Education changing?
  • What kind of "big numbers" has Frank been impressed with lately? (spoiler: it has to do with Spear Practice Solutions)

Overtreatment. Does it happen in dentistry or is it just a difference of opinion? In today's "ask us anything" episode Jason and Al address the topic of who's treating too much and who's not treating enough?

  • The case of a new dentist diagnosing "10 cavities" in a patient that's experienced no decay with their old dentist
  • The case of a "second opinion" with a giant perio treatment plan and no pockets over 3mm
  • Can the customer service model of "Discount Tire" be applied to dentistry?
  • Which would you rather repeat: dental school or 2020?
  • How much does a missed phone call cost? Does it make sense to work with someone to answer your calls "off site?"
  • Do you diagnose and treatment plan the same at the beginning of the day as the end of the day?

In the new COVID landscape some things are going virtual for the first time. For instance, take the ADA Meeting. It’s now the “ADA Virtual Connect Conference” and it’s coming up October 15th through the 17th. More about that in the Dental Hacks Nation Facebook page! YAPI has been helping dentists go totally digital for a decade now for all kinds of tasks throughout the dental office. You’ve got to take in and organize a ton of patient information and make sure it shows up in the right place at the right time. You’ve got to be able to communicate throughout your office at any time, yet you need to do it discreetly. As much as you wish you didn’t have to...you need to remind your patients of their appointments and try to avoid broken and failed appointments. You have to reactivate the patients that have fallen through the recall cracks. And now more than ever before you have to be able to ask your loyal patients for reviews in the right places like Google, Facebook and Yelp. It’s all important and needs to get done, but it can be a drag on your team. Our friends at YAPI have solutions for all of this and more! And they can do it paperlessly! To be honest, there really isn’t much in your office that YAPI can’t make easier and more efficient. Check out all that YAPI can do for you at dentalhacks.com/YAPI.

Stephanie actually interviewed Alan for her podcast "Dental Outliers" and we thought it was so good that we're featuring it here! Jason and Alan got a question emailed to them from a new dentist who is struggling with their current associateship. The email asked:

I'm a recent graduate from dental school and just got a new associate position at a private practice near my hometown.  I've been working there for about 2 months now and the patient flow for me has been very slow--seeing 0 patients on some days.  The owner dentist has been doing some marketing for myself to get going and most new patients that walk through the door are mine to be seen.  What else can he and I do to improve my patient flow?

The owner dentist is also big on continuity of care and doesn't allow me to see his patients.  However, he and I had talked about potentially buying into his practice somewhere down the road.  Since this is a long-term relationship, should I be seeing some of his patients?  Should I be doing hygiene exams, simple restorative work, some crowns, new patients and emergencies? What are your thoughts on continuity of care and how does a new associate fit into that?
As usual, the Hacks have thoughts! Head on over to the Dental Hacks Nation to join in the conversation!

Alan had a chance to talk with Joshua Scott of Studio 8E8. Joshua has around 20 years of experience in dental marketing and he realized that dental marketing sucks. It's a harsh truth, but totally true. We discuss all about marketing in dentistry:

  • Authenticity and storytelling in the dental space (Al is a Seth Godin fanboy)
  • How dental marketing USED to work
  • What is Joshua's hack to reduce expenses that you might not be using? What's his 24 hour rule?
  • Do patients see dentists as a commodity? Can we change that?
  • How customers see your office and creating an experience
  • Telling the story of safety isn't going anywhere
  • What does storytelling mean to Joshua?
  • How important is SEO? Reviews?
  • What's the best creative medium? Why does Joshua feel like you need to do all of it instead of focusing on one type of medium?
  • Dental stock photography is horrible
  • Startup marketing vs. existing practice marketing
  • How important is branding in dentistry?

Jason and Alan are joined by Dr. Mark Costes and Dr. Brad Keener for a really interesting talk. As it turns out, both of these friends of the show survived accidents that left their faces and teeth smashed up. Both of them have been pretty open about what had happened to them in the past, but we thought it might be interesting to hear about what it was like to be a patient of a big reconstruction after traumatic accidents. And we were right! This is an interview that you aren't going to want to miss!

On this week's clinical hacks podcast,  Dr. Zach Meiners bring back Dr. Daniel Smith and Dr. Rusty Fitton to talk more about practicing with a parent or family member.

What archaic equipment do our '70s and 80s parents still utilize in the dental office?

What would you do differently if you could give advice to your younger self?

What should young dental students know if they are planning to join a parent in their office?

And much much more.

Are you planning to practice with a relative?   Join us in the Clinical Hacks Facebook group just for docs to discuss it further!

Until now! Sarah explains how she came up with the brilliant idea of Stop Gap Dental, a smartphone based app that wants to be your one stop solution to this problem (and a whole lot more)! Find a qualified sub at the time you need it? The app can do it. Pay them? The app can do it! Sarah explains how Stop Gap Dental could work for your practice!

Dr. Josh Austin and Dr. Lance Timmerman join Jason and Alan to talk about their new podcast, Working Interferences with Josh and Lance.

You know what would be a great idea? A podcast that gives advice to dental people. Or not so great advice. Either way, really. Dr. Josh Austin and Dr. Lance Timmerman have just the thing! The Hacks and the Interferences go through some "Working Interferences" style advice and it's a blast! Beware...it gets pretty ugly. Perhaps the most editing for content of any Dental Hacks podcast in the past!

The trilogy of episodes with Dr. Nate Lawson comes to its conclusion.   We'd like to think this one is more "Return of the King" than "Spider-Man 3"  but we'll let you be the judge of that.

In this episode Dr. Lawson waxes poetic on bioactivity.  What is it?  If these bioactive ions are released and no one is around to see them, did it really even happen?

We discuss Activa, Glass Ionomers, pulp caps and liners, and much much more.

In today's throwback episode (originally presented May 19, 2017), Dr. Dennis Hartlieb joins Jason and Alan to talk about his journey from believing composite was a "temporary material" into creating an entire rehab in composite!

Dr. Dennis Hartlieb was one of Dr. Frank Spear's original students. He's been learning excellence in restorative dentistry for 25  years or more. But like many dentists, he used to believe composite had no place in long term restorative dentistry. Then he met Dr. Buddy Mopper and he began to change his mind. Dennis is a really engaged teacher. Really approachable and seems to really understand how dentists learn. Some highlights:

  • Dennis's approach to team bulding...learn one, do one, teach one!
  • Simple composite solutions that can make a huge difference!
  • How does sleep apnea affect restorative dentistry?
  • A description of Dennis' course at Cosmedent, "CPR for the worn dentition."

All joking aside,  Frank waxes poetic on predictability and finding your happy place in dentistry.  Being the decathlon dentist isn't for Frank.  He likes to crush on things he's great at.

This "greatest hits" episode features two of our greatest Brain Trust segments, both recorded from Dentsply Sirona World 2018. Do you all remember a time when lots of people could get together in a convention center and look at exhibitors and see speakers? Yeah, me too.

Jason and Alan spent 3 days podcasting from the Spear Education booth at Dentsply Sirona World. Because the Dental Hacks are all about value, today's episode includes not one, but TWO amazing Brain Trusts recorded with a bunch of all stars in a double length podcast episode! We're here to 10x your podcasting experience!

The first Brain Trust features Dr. Rich Rosenblatt and Dr. Darin O'Bryan. They're best buddies and both of them are CEREC rock stars! Highlights include:

  • Rich's experience and tips on starting a study club
  • How to bring in an associate
  • Dental school horror stories from mid-90's University of Minnesota

The second Brain Trust features podcast newcomer Dr. Mike Skramstad (CERECdoctors rock star!), Dr. Craig Spodak (of the Bulletproof Dental Practice) and Dr. August de Oliveira (of Digital Enamel) in a hilarious round table including:

  • Will impression material ever be obsolete?
  • What's coming down the pipeline for CEREC (hint...it involves hovercrafts)
  • Firewalking with Anthony Robbins
  • Craig's new book: "The Bulletproof Practice"

You're going to love this double length episode. And if you don't, we're happy to give you a full refund! That's how we 10x your podcasting!

Only one Clinical Hack showed up today so Zach Meiners recruited Shawn Van De Vyver to come out and play.

Shawn is a computer guru and drops some knowledge on how to get some virtual consulting done at your office to talk to new and existing patients who might be reluctant to leave the house.

As always there's an expensive way to add virtual consulting to your website and a cost conscious one and Shawn is delivering the affordable with Ninja Forms and Zoom meetings.

We also discuss our favorite old school NES games that we had been playing during the lock down... what are your favorite games?

Don't forget to check out the Clinical Hacks Facebook page for further discussion!

  • Where is the ADA on all of this?
  • Insurance verification...it's pretty important right now, especially eligibility
  • Membership plans
  • Delta prepayment for cash flow (what's the catch)
  • What does it look like to drop Delta Premier?
  • Using COVID as an excuse
  • What about a second wave of COVID?
  • Why is Jason so damned handsome?

It seems like everyone is talking about aerosols. The Coronavirus crisis has focused our attention on the possible spread of the virus in dental offices and specifically what we can do to reduce it and keep our patients and teams safer. We don’t have all the answers yet, but we do know that suction evacuation and rubber dam is a sure way to reduce dental aerosols. Our friends at Zirc have been producing some of the best products in these areas for a lot of years. If you aren’t familiar with the Zirc family of isolation products, they want you to take a look. You can get the Zirc “Protection Pack +” trial kit for only $9! The protection pack includes their amazing Mr. Thirsty One step (1 Pedo/small and 1 Medium/large) size. It’s the bite block high volume suction evacuator all in one that doesn’t need any complicated hardware. Just plug it directly into your high volume suction and you’re ready to go! The Protection Pack + also includes three “pink petals” which turns any saliva ejector into a hands free suction machine! And finally you get two of Zirc’s amazing Insti-Dam! We've all got questions about what's going on in the world. And the Dental Hacks have answers! We don't guarantee that they're good answers, but they are answers. Jason and Al answer questions from the Dental Hacks Nation talking about what's happening in our offices, new PPE, cartoons of our youth and who is better at spelling (spoiler: it's Al).

Some things we discussed:

  • Do you really think dental offices will ever return to what they were B(efore) C(orona)?
  • Is it hard to stay true to your values and avoid moderating the Facebook group?
  • Who looks better with a mullet?

The Insti-Dam is a nonlatex dental dam and frame all in one! Pre-punched for that emergency endo that shows up in your office tomorrow! Go check out the Protection Pack + at dentalhacks.com/protection and try one out for the low price of $9!

We know this has been an incredibly trying few weeks for really everyone, our dental friends and their patients. Many dental teams are just scratching their heads wondering, what should we be doing? There’s no book on this stuff, well not yet at least, but you know what? We have some advice for you, and it’s coming from our friends at YAPI. All of you that know of or use YAPI are likely familiar that one of their founders is Dr. Gina Dorfman, a practice owner herself. Dr. Gina and YAPI’s marketing team are going to help you stay productive and stay positive during this time. Whether you are closed altogether, treating emergency patients only, we’re going to be sharing a variety of different resources that will help you stay on top of this issue and how to stay engaged with your patients and make sure you don’t miss any opportunities that come your way. As COVID-19 survivor Tom Hanks once said, There’s no crying in baseball, YAPI says there’s no downtime in dentistry! Alan talks with Dr. Craig Harder about his recent news exposure! Craig has been printing reusable masks for health care professionals in his town in Washington and has had several great news stories written about his efforts! Craig tells us how he's printing them and how he's customizing them for nurses and doctors in his area! We talk about how to print a 3D mask as well as whether our existing universal precautions were enough. Infection control in dentistry has always been a huge deal and we wonder if people are going to criticize what we've been doing!

So it looks like we're going to have some down time. Our friends at Zirc understand this and have a great idea for you! What if you used your down time to work on your clinical systems? It's useful and it's low risk! Zirc has a process that they call ZOBE. That stands for Zirc’s Organizational Box of Efficiency. They send a big box full of their different instrument organization options to YOUR OFFICE. You get to try it all out. Snap the tops on the tubs. Put cassettes in your own sterilizer. Try out YOUR instruments in whatever cassette you like! Plus, Zirc sets you up with your own organizational expert like Kay to help you determine what’s right for you. Most importantly, the ZOBE process is FREE. It doesn't cost you anything to learn about Zirc's clinical systems. It's a perfect thing for you to do while you've got some down time! Plus, if you do a ZOBE consultation by April 17, 2020…you’ll be in the running for a $300 Amazon gift card! Find out more at dentalhacks.com/ZOBE!
