The Very Dental Podcast Network (dental)

Have you ever wished you could go to dental school as a dentist? Wouldn't it be great to have the experience and perspective that you have now when they were trying to teach you the basics? Jason and Alan talk to Dr. Gregg Spear of Spear Education about this very concept. When we were being taught about occlusion in dental school we didn't really understand why we were learning this nor how it affects our treatment. Jason and Alan were at Spear Education taking the phenomenal Occlusion Workshop and talked with Gregg about how Spear kind of ends up being "dental school for dentists." Some topics that we cover:

  • Dentists as introverts
  • Becoming a teacher
  • Spear Summit
  • How Spear Education instruction has changed over time
  • The "red pill" aspects of airway and joint issues
  • Work-life balance
  • magnification and dental microscopes
  • light sources
  • Incorporating digital workflow at Spear
  • Saving teeth vs. placing implants
  • What would you tell a first year dental student? What would you tell a 4th year student?

Welcome to season 1, episode 15 of the Clinical Hacks Podcast

This week we gave Mac Jones the week off to build a patio table and instead we talk about sealants.

Zach Meiners, Kevin Fryer, and Alan Mead have one thing in common, we all pretty much dislike sealants!    What?  Aren't they preventing decay?     Enjoy what's likely an unpopular take on sealants.

Highlights include:

- Do sealants prevent decay?

- Can sealants "cause" decay?

- What conditions should be met for sealants to be effective?

- Is adding sealants into your hygiene program a revenue booster?

- When did 3 experienced clinicians turn sour on sealants?

- How does a microscope user view the sealant restoration?

- Is the medium quality sealant the worst restoration?

- Glass Ionomer sealants, do they have a place?   Fuji Triage

Everyone talks about delegation. It sounds great, but it's not as easy to do as you would think. Dr. Gina Dorfman is a person who gets a lot of things done. We talk about a lot of things in this short podcast. Here are a few of them:

  • Perfectionism isn't a good thing. On the whole, it's a bad thing. It keeps you from starting because you might not make it perfect.
  • Done is almost always better than perfect.
  • Al gets therapeutic about podcast production
  • Do things that you like to do the most. Not only that you're good at, but you ENJOY doing.
  • Finding the right people to delegate to is worth it
  • Why being on Facebook isn't always a waste of time
  • Finding associates on the beach in Hawaii
  • Training is the most important part of delegation
  • If everyone always disappoints you, it's probably you
  • Can your office run without you?
  • Are you a micromanager?

Jason and Alan sat down to talk with Dr. Darin Dichter from Spear Education. They were taking the 3-day Occlusion Workshop and Darin was one of the instructors. This wide ranging discussion is spectacular and covers:

  • Occlusion philosophies
  • Dental School
  • Continuing education and the struggle to implement ("getting reps")
  • Student loan debt
  • Not all dentists are clinicians!
  • Critical thinking and understanding the literature
  • Being a nontraditional student and perspective outside of school
  • What is a terminal dentition, when do you know how to treat it and what's the "Treating the Terminal Dentition" course all about?

Dr. Alan Mead talks about his experiences with difficult patients and offers a few tips.

  • Find the red flags before you treat
  • Patients can’t/don’t/won’t understand treatment...what can you do about it
  • Create a relationship and find out when it’s less about your or your treatment and more about their life.

Welcome to season 1, episode 12 of the Clinical Hacks Podcast

The gang is back.  Kevin, Mac, and Zach discuss posts and cores

Highlights include:

- When do you choose to place a core, or a post?

- Do you remove all the previous restoration every time?  Most of the time?

How does Mac do a core?

- He mentioned like 5 products, Cosmecore, Absolute Dentin, Fuji 2, Equia Forte, and Miracle Mix.  I'm exhausted just typing all of those, so I pinned him down to one for technique.

- Cosmecore

- Mac bonds that in with a Self-Etching Primer Bonding Agent Prelude SE (technically he uses the generic KoolBond SE, but uses the Prelude branded dual-cure link)

How does Kevin do a core?

- He uses regular light cure composite sometimes (Kalore from GC), but usually uses Absolute Dentin from Parkell

- Bonds with Brush and Bond from Parkell

How does Kevin do a post?

- Dentatus Posts

- Buildup material cements the post

How does Mac do a post?

- Unicore Posts

- Buildup Material cements the post

Zach buildup and posts:

- Bulk EZ for buildups (autocure)

- Aforementioned KoolBond SE (generic of Prelude SE) to bond.

- Parapost XP

- FibreKleer Posts

- Vlock Posts (now Edenta Headmaster)

Jason and Alan are joined again by the legendary Dr. Mike DiTolla! In this wide ranging conversation we talk about all kinds of stuff. Mike claims to be a charter member of "Underpreppers Anonymous." He's seen a lot of tooth preps in the world's largest dental lab over the years and the bottom line is this: dentists routinely underprepare teeth. Particularly molars. One of Mike's goals in life is to help dentists do it right. If you prep crowns, this is a valuable discussion about preparation techniques, intraoral scanners and tools for more ideal reduction. We also discussed the future of hands on dental CE and bigger dental meetings. Finally, we talk about what's happening in the world of full contour zirconia (including bridges) since Mike was on the ground floor at Glidewell when Bruxzir was being developed!

Alan and Dr. Jason Smithson discuss what "group practice" means. The concept of group practice is much more common in the UK. Jason and Alan discuss what it means in each place and why there isn't more group practice in the U.S. Jason has actually worked in multiple "group practice" situations and tells some stories about how it can work some ways better than others. They also challenge the concept of "chair hopping" and the "excess capacity" and the flaws of these concept. This discussion is really helpful and kind of inspiring!

"You get the rough with the smooth."

--Dr. Jason Smithson on having a specialty

Alan and Jason continue their conversation about specialization. This episode is where we get to the good stuff where we really dig into the pros and cons of specialization and how generalists that long to be known for a particular kind of work might have it wrong! Jason debunks some of Alan's preconceived notions about how having a referral base makes a patient choosing treatment almost automatic. And Alan is mildly crushed.

Also we discuss the excruciating problem of having a 20+ year old practice/patient base and the ups and downs of having a "history" with people. Finally, we open the door to a potential next podcast to talk about an ideal way to do group practice.

This is a really good one. Don't miss a minute!

Dr. Jason Smithson joins Alan this week to talk about specialization. Jason has taken a pretty interesting path to the career he currently has. His first focus was going to be going through a fixed pros residency in the US with a plan to teach at a University in London along with having a cushy private practice gig to boot. Then things changed and Jason began to train in oral surgery, but once he realized the commitment he'd be making to more education, he switched plans again. He found himself in a General Practice Residency and actually stayed in that practice for upwards of 20 years!=== While he was here, he actually began training in a perio residency as well. Jason is a good person to discuss specializing with, but I think you'll find this conversation goes in directions you aren't expecting. We'll follow this conversation up with the second half next week!

So many dentists become business owners almost by accident. Yes, we expect to be high earners, but many of us don't really know what that means. We're highly trained in dentistry but we're essentially untrained in handling money. Reese Harper of Dentist Advisors understands this. His advice based financial planning services helps dentists understand the kinds of things that they should be doing and should be outsourcing to people that know better.

As the Dental Hacks approach 5 years in podcasting we are featuring one of the original members of the Dental Hacks Brain Trust, Dr. Matt Standridge! A lot has happened in those 5 years for Matt and we dig into what he's been up to in that time. For one thing, the dude has lost a lot of weight and become kind of a big deal in the low-carb/keto/primal diet internet space. He created a podcast a year or so back that allowed him to really get to know the movers and shakers in this movement! And if that wasn't enough, he's become a kind of ortho guru, too! He's an instructor with Gerety Orthodontics as well as begun working with Blue Sky Bio on some pretty cool stuff (indirect ortho bonding anyone?)!

We had a wide ranging conversation with Lou that covered a bunch of topics:

  • What's new with Spear Education and the new endo curriculum online and on campus.
  • Lou tells us the amazing story of the first year of Berman Instruments!
  • What innovations should we expect in the near future in endodontics?
  • Lou gives us his take on the Atlantic article that's gotten under Al's skin so much.
  • How can dentists make sure they understand science and research better?

Welcome to our new podcast that we're calling "Smithson on..." The podcast will feature one of our most popular returning guests on the Dental Hacks podcast, Dr. Jason Smithson. Jason is a clinical dentist and educator from the UK. Dental Hacks co-hosts Jason and Alan met at a Jason Smithson course at Cosmedent in Chicago in September of 2013 where the idea for the Dental Hacks podcast was hatched. You could say that Jason Smithson had a big effect on us and we're glad that he's going to be part of the growing Dental Hacks Podcast Network!

One thing we've learned about Jason is that he's a great storyteller. Add to that his mastery of restorative dentistry and his knack for teaching it to other dentists and you've got a pretty compelling guy. Our new podcast will feature conversations between Al and Jason (Smithson) on focused topics, most often related to clinical dentistry. I think you'll see that it's a pretty great listen.

We're kicking off our first episode with a discussion of science and the somewhat twisted relationship that it has with dentistry. If you have any questions or comments about the episode please email us at info@dentalhacks.com or better still, post it on the Dental Hacks Nation Facebook group!

We'd like to welcome Jason to our podcast network and we're glad he's here!

Dr. John Khademi and Dr. Eric Herbranson join Alan to talk about the state of endodontics, conserving dentin and how we know what we know.

I got to know Dr. John Khademi in the mid-2000's from Dental Town. I was introduced to Dr. Eric Herbranson in 2011 at the Academy of Microscope Enhanced Dentistry where he talked about the 3D Tooth Atlas he had created. Both are influential endodontists who have controversial ideas about how we've been treating teeth over the years.

Today's episode features a journey from patient based outcomes to case selection, access openings to how we know what we know. There's a fair amount of hero worship here I've been a huge fan of these guys for quite some time. And a bonus...the one and only Dr. David Clark of Bioclear sits down at the end and talks endo!

If you like endo (or even if you don't), this interview will change the way you look at saving teeth!

Vacation. Time off. Holiday. Whatever you call it, we all crave it and we want more of it. But how does it work in your office? We decided to have a Brain Trust to cover "how we take time off" and it was amazing!

Ironically, Jason was on vacation when we recorded it. But joining Alan for the first real Brain Trust in a while was Dr. Brent Young, Dr. Frank Clayton, Dr. Tammy Bailey and Dr. Kevin Fryer! The conclusions that we came to is that many offices do it similarly, but none do it the same! In this NSFW Brain Trust you're going to learn some stuff, but more importantly you're going to laugh!

"We as dentists make life hard for ourselves by compromising too much."

--Dr. Jason Smithson

In a nutshell, this statement is why Jason Smithson is my hero. Also my spirit animal. In this very special edition of the Dental Hacks Daily I had a chance to talk with Dr. Jason Smithson about the rubber dam. Jason is known for his exquisite restorative procedures. And one of the many things that makes him such a fine clinician and teacher is his use of the rubber dam. I spend this episode asking Jason all about it. We discuss:

  • What does he do for patients that don't tolerate it?
  • What's the best way to explain rubber dam to a patient?
  • What does he think of Isolite and other bite block/suction style options?
  • Are there any procedures that he thinks are inappropriate for the dam?
  • Who places the dam in Jason's office?

This episode gives the listener a sample of another new podcast that will be coming out on the Dental Hacks Podcast Network! Coming in May 2019, the "Smithson On..." podcast will be conversations and rants featuring the wit and expertise of Dr. Jason Smithson!

Dr. Dennis Hartlieb has created what we think is going to be the future of dental CE! He's one of the best teachers of restorative dentistry anywhere and he has created this amazing dental continuing education platform that he calls "Dental Online Training" and he wants you to try it! Go check out dentalhacks.com/dot and sign up for his comprehensive digital version of "CPR for the Worn Dentition" for free!

It's all about convenience. Talking on the phone is an OLD technology! It's time to turn your social media presence or website into a dental version of Uber! Why haven't you taken the Neodiamond Challenge yet? Microcopy Dental thinks that their single-patient-use burs will stand up to any comparison of multiple use diamonds without the hassles of re-sterilization and all the hassles that go along with it. They want you to head over to dentalhacks.com/challenge, request a sample of their most popular crown preparation diamond, the 1116.8C, and put it to the test! We think it's going to change your mind about diamond burs!

Proud isn't really the right word. Maybe ecstatic is.
The Dental Hacks podcast has a spinoff! Just like "Happy Days" produced "Mork and Mindy" and "Breaking Bad" birthed "Better Call Saul," the Dental Hacks podcast is proud to announce a new podcast. It's based on the spinoff Facebook group that we call "The Clinical Hacks" and we're sticking with that name for the new podcast!

You might remember that we did a round table about endodontics back in episode 223 that featured Dr. Kevin Fryer, Dr. Zach Meiners and Dr. Mac Jones and there was a bit of magic there. These three "regular ass" dentists love talking shop and that's just what you'll hear on the Clinical Hacks podcast!

In today's episode we talk with Kevin, Zach and Mac about what they hope to accomplish with the podcast and what we can expect!

It's all about convenience. Your patients want to make appointments for your office on their own time, and they don't really need to talk with your people! Our friends at Localmed can make that happen no matter where your patients are looking for you! Google, Facebook, Healthgrades and even Yelp...Localmed has you covered! Jason and Alan welcome Dr. Frank Spear to the podcast to talk about some big picture concepts. Frank was asked to give the keynote lecture to the Cal-lab Annual Session in 2019 and was asked to predict the future of dentist/dental lab relationships. So he dug into a lot of statistics and research and now we're going to share some of that with the Dental Hacks listeners! Some of the things we talk about with Dr. Spear:

  • Why have the number of dental labs been shrinking?
  • Where have all the lab technicians gone?
  • Is the narrative about corporate dentistry taking over accurate?
  • What happened in the mid-70's through the mid-80's that helped create a particularly large cohort of dentists that are now reaching retirement?
  • Is the need for dentists growing or shrinking?
  • How will technology help us with treatment planning?
  • How did he transition from traditional perio/pros to implants and how did that transition go?
  • What has made implants a more predictable treatment option starting in the 90's?
  • Are we seeing dentists move too quickly to implants? Will there be a backlash?
  • What happened to the mustache?

We appreciate the time that Dr. Spear took with us and we think you'll really enjoy this episode! If you like the episode, would you please tell your friends about it?

Alan sits down to talk with Ben Rojek, of Spear Education. Ben wears multiple hats at Spear and one of them was the editor of Spear Digest! He has a background in journalism and marketing and we had an interesting conversation about content creation for dentists!

  • How is Spear helping dental students start their careers with affordable continuing education?
  • How has the "content creation machine" that is Spear Education changed in the past several years?
  • What's it like to be the editor or one of the most widely read dental blogs anywhere?
  •  What are some easy things that regular dentists can do to be found online?
  • Why should you start a blog? (spoiler: Al has thoughts)
  • How should dentists respond to reviews?
  • What are some great ways to portray yourself and your team on social media?
  • How can Spear help you with patient education?

  • What is case acceptance?
  • Why did your patient come into the office in the first place? (spoiler: they probably don't know)
  • What are some ways to add photography to your office? Why should you?
  • Where should you put your camera in your office? (hint: it's not in your personal office)

We bring it back to the beginning with episode 233. Way back in June of 2014, Jason and Alan launched the Dental Hacks podcast with our first guest, Dr. Lance Timmerman! You may know Lance from the wildly successful and funny "Working Interferences" podcast or you may just know hi as the dentist with the world's best head shot.

We're not polished. We're not very sure of ourselves. Yet, it ended up being a pretty great interview! So...we take you back to the beginning. The show that started it all! And we'll see you next week with some amazing brand new content!

Jason and Alan are joined by Nicolle Campion (president) and Kay Hickey (sales specialist) of Zirc. You see, it just so happens that Jason is doing another startup and our friends at Zirc are here to explain that it's never too soon to be planning your clinical workflow and organization! They explain how Zirc makes this easy with their ZOBE (Zirc's Box of Organizational Efficiency) product! How great would it be to have a bunch of different organizational products shipped right to your office so you can hold them in your hands and see how they actually feel while getting coached by the organizational experts at Zirc? It's a great idea and you should try it out! You can find out more at dentalhacks.com/zobe!

Every dentist has their own bur block. You should fill yours with single patient use Microcopy diamonds and carbides! For instance, you'll find a ton of uses for the 3314.10 very fine pointed cone trimmer. I know I do. I use the "skinny" to break contacts without nicking up the adjacent tooth as well as for finishing flat-ish surfaces like the facials of upper and lower anteriors! Give it a look at dentalhacks.com/skinny.

Sure it's fun to roll out the newest intraoral scanner or 3D printer. Not every product can be that sexy even though they can be important! Dr. Jason Goodchild joins Jason and Alan to talk about the Chicago Midwinter, how to time product roll outs and some new products from Premier!

Traxodent and the special clay

As much as I love Microcopy single patient use diamonds, I love their carbides, too. Especially the Neo Max "Max 2." When I have to remove a PFM or gold crown or even cut through amalgam, this is what I reach for. It slices through metal quickly and easily...just like butter. Go check them out at dentalhacks.com/neomax.

Jason and Alan had a chance to talk with fourth year dental students Timothy Johnson and Colin Mitchell. They're finishing up dental school at Southern Illinois University school of dentistry. These level headed D4s filled us in on what's good, bad and ugly in dental school these days.

Have you ever had a patient swallow a crown? Feels pretty great, right? How about you put all of that behind you by trying Airway Armor from Zirc! If you order on their website at dentalhacks.com/airwayarmor and use the coupon code: DHSAMPLE and you'll get samples of Pink Petal and Mr. Thirsty One step along with your Airway Armor trial kit!

There's something to be said about a product that you use every day. MirrorImage from Cosmedent is one of those products for me. Go check it out at dentalhacks.com/mirrorimage.

I promised Jason that I wouldn't put this one out.

Promise broken.

Jason and Alan decided to do an impromptu sound check podcast at the Premier Booth at the Chicago Midwinter and the result was hilarious.

If we have any Russian listeners please let me apologize in advance for our horrible accent. We discuss the stuff we were excited to check out at the Midwinter, the McCormick Center security team, Jason's discomfort with women and endodontics and Alan's inability to fit into black lab coats.

Mila Ugryn is the social media guru at Cosmedent. Jason and Alan had a chance to speak with her about how dentists can use social media to promote their practices. Specifically...Instagram! She gives a bunch of great advice to dentists trying to start on Instagram! Her most important advice: "just start doing it." Consistency and authenticity is the key to social media!

If you sign up for the March 14-15th date, you'll get the online version of the course (being recorded on March 14-15!) for free! Go check it out at dentalhacks.com/Corky!

1983. The beginning of seventh grade. That's when the Rich and Al story started.

Al has been friends with Richard Heend for more than 35 years. Which makes me feel super old. Rich had computers long before people normally had computers. This is where Al's fascination with computers and being "online" started.
