Jason and Alan had the privilege to speak with Dr. Rebecca Bockow, a dual trained orthodontist/periodontist and cutting edge advocate for understanding airway. Rebecca has been teaching at Spear Education for a couple of years and will be one of the instructors in the newly revamped "interdisciplinary treatment planning" course that Spear is putting together. Jason and Alan had the privilege of sitting down with the one and only Dr. Frank Spear. We had an hour during the Spear Summit to pick up where we left off from episode 235 and it was very interesting. Frank opened the meeting with a talk that discussed the stories that dentists tell each other about where the profession is going and then presented some data that suggests a lot of these stories aren't exactly true.   • Could someone become Frank Spear in the way that Frank did considering student debt as it is now?
  • How did beautiful temporaries make a difference in Frank's career?
  • Why does Frank feel responsible to help the narrative that dentists tell each other?
  • What's the data the Frank would really like to see?
  • Why is it so impossible to compare dental businesses and income across the country? How can you compare apples to apples?
  • Take home message: things are better than dentists are telling themselves!
  • What is dental school like when your dad is Frank Spear?
  • How could dental school teach things with more context?
  • Are there useful parallels between dental and medical education?
  • How will Frank's teaching schedule be changing in the future?

Alan has talked about (bragged about?) The Francis B. Vedder Society on the show for years. He's been a member for 20 years or so and it's been very influential in his career. Some of the speakers Alan has seen in the last 20 years or so include Dr. Frank Spear, Dr. John Kois, Dr. Jeff Rouse, Dr. Jeff Okeson, Dr. Stephen Chu, Dr. Pat Allen, Dr. John Burgess, Dr. Bernard Touati and Dr. Bob Winter to name a few. It's been a literal "who's who" in dentistry lecturing to a small group of colleagues in a laid back setting.

Last weekend was the 2019 Fall meeting and the society celebrated 60 years of continuing education and fellowship. Alan recorded a "live podcast" at the event where many of the members of the Vedder society shared some funny and emotional stories of what the society has meant to them. Everyone talks about delegation. It sounds great, but it's not as easy to do as you would think. Dr. Gina Dorfman is a person who gets a lot of things done. We talk about a lot of things in this short podcast. Here are a few of them:

Alan and Dr. Jason Smithson discuss what "group practice" means. The concept of group practice is much more common in the UK. Jason and Alan discuss what it means in each place and why there isn't more group practice in the U.S. Jason has actually worked in multiple "group practice" situations and tells some stories about how it can work some ways better than others. They also challenge the concept of "chair hopping" and the "excess capacity" and the flaws of these concept. This discussion is really helpful and kind of inspiring!

This episode is where we get to the good stuff where we really dig into the pros and cons of specialization and how generalists that long to be known for a particular kind of work might have it wrong! Jason debunks some of Alan's preconceived notions about how having a referral base makes a patient choosing treatment almost automatic. And Alan is mildly crushed.

Also we discuss the excruciating problem of having a 20+ year old practice/patient base and the ups and downs of having a "history" with people. Finally, we open the door to a potential next podcast to talk about an ideal way to do group practice.

Dr. Jamison Spencer joins Jason and Alan to talk about sleep medicine and TMD treatment in the dental office.

Once you’ve learned some things about sleep apnea you come to see the symptoms and consequences in LOTS of your patients. Bruxism, gastric reflux, TMD and lots of other conditions are correlated with sleep apnea. Dentists are quickly becoming the most likely screeners for this potential deadly condition, and Dr. Jamison Spencer is one of the most sought after lecturers on the subject. Dr. Spencer joins Jason and Alan to explain how to incorporate this treatment into your practice, what symptoms to screen, what gadgets NOT to buy and what diagnoses you shouldn’t be making! Dr. Jason Smithson joins Alan this week to talk about specialization. Jason has taken a pretty interesting path to the career he currently has. His first focus was going to be going through a fixed pros residency in the US with a plan to teach at a University in London along with having a cushy private practice gig to boot. Then things changed and Jason began to train in oral surgery, but once he realized the commitment he'd be making to more education, he switched plans again. He found himself in a General Practice Residency and actually stayed in that practice for upwards of 20 years!=== While he was here, he actually began training in a perio residency as well. Jason is a good person to discuss specializing with, but I think you'll find this conversation goes in directions you aren't expecting. We'll follow this conversation up with the second half next week!
