I’ve found that using a digital workflow for posterior zirconia is very predictable. So much so that when I’ve evaluated margins, contour and interproximal contacts...I’m ready to cement. Why so cocky? Typically the digital workflow usually nails the occlusion. But more importantly, Microcopy Dental has a posterior adjustment system for zirconia (and lithium disilicate) that is so simple and SO GOOD. First, I use Bite-Chek articulating paper for accurate and easy to read occlusal contact marking. Then I use the specially designed Z adjuster diamond (my favorite is the tear drop shape) to refine my occlusion and finally I use the 2 step Neoshine Z polishers to bring my surface back to a high, polished shine! Microcopy has done the heavy lifting to make adjusting posterior restorations a breeze. And all of their products are single patient use so it’s easy clean up and no cross contamination! Go check the system out at verydentalpodcast.com/adjust!

Ideal preps. That’s what we aim for. One excellent way to get there is to use a core material to build the tooth back up to ideal contour. Cosmecore from Cosmedent is my choice for building up a broken down tooth. It’s dual cure, comes in a variety of colors and cuts just like a tooth. You’ll get a super smooth finish with no gouging. Right now I’ve got the 50 gram dispenser that fits in a small impression gun, but when I need to refill I think I’ll go with the 8 gram syringe. Both work great, but those syringes are handy and don’t take much room on a set up. Check it out at verydentalpodcast.com/cosmecore!

It’s funny, but you never really know how your branding effects people until you see it in your own life. So Jacob, my youngest 11 started Invisalign with a local orthodontist. He’s following all the instructions, including taking them out to eat. So I’m CONSTANTLY seeing a case for his aligners with our orthodontist’s brand. Like every single day. I’m realizing that cases for aligners, splits and removable offer a brilliant way for people to remember your office and your brand! And then I thought of our friends at Zirc who offer aligner sized and denture sized boxes in tons of colors that you can customize with YOUR logo! They’re offering a buy one, get one deal on retainer or denture boxes with your logo and 4 lines of custom print! Go check it out at verydentalpodcast.com/imprint and use code: verydentalimprint for your Very Dental deal!

Our friends at Enova Illumination have your magnification and lighting needs all figured out. Alan features a throwback interview with Dr. Frank Spear on this Black Friday episode! Back in 2019, mere months before the pandemic, we had a chance to talk with Frank Spear about some interesting research he was doing. It seems that what dentists thought about the profession isn't exactly what the data suggested about the profession. Some topics that we covered:

  • Why do insurance companies cover thing so differently from state to state?
  • The beauty of practicing in groups. But not necessarily a DSO.
  • Could someone become Frank Spear in the way that Frank did considering student debt as it is now?
  • How did beautiful temporaries make a difference in Frank's career?
  • Why does Frank feel responsible to help the narrative that dentists tell each other?
  • What's the data the Frank would really like to see?
  • Why is it so impossible to compare dental businesses and income across the country? How can you compare apples to apples?
  • Take home message: things are better than dentists are telling themselves!
  • What is dental school like when your dad is Frank Spear?
  • How could dental school teach things with more context?
  • Are there useful parallels between dental and medical education?
  • How will Frank's teaching schedule be changing in the future?
  • How is the curriculum at Spear Education changing?
  • What kind of "big numbers" has Frank been impressed with lately? (spoiler: it has to do with Spear Practice Solutions)

Herodontics.

For normal, mortal human dentists, sometimes we do it. Even when we know we shouldn't. But can our patients really understand it? In today's episode Alan finds an analogy that helps based on the college basketball game he recently attended!

Alan welcomes two of the most accomplished and entertaining presenters in dentistry to the Group Function podcast. Dr. Mike DiTolla and Dr. Josh Austin are known as dental educators that can connect with an audience better than anyone and they prove it again in today's episode. Both of them have been working out new and never before seen material for the Voices of Dentistry meeting coming soon in January 2021! Some of the topics they covered:

  • The very first Voices of Dentistry! (Holidome anyone?)
  • Josh and Mike have taken VoD "under their wing." It's their favorite meeting to present at. And we love it!
  • Previews for VoD
  • Why is social media is so bad for us?
  • How do Mike and Josh prepare/write their stuff?
  • "Start with the 3 most embarrassing moments of your life..."
  • May be an image of one or more people, beard, people standing and text that says 'MICHAEL DITOLLA JOSHUA AUSTIN ONCE UPON TIME IN... SCOTTSDALE AMPI -MEMBER'

Nate Lawson, DDS is a dental researcher who is the mastermind behind the @dentaltube Instagram page which has been voted one of the top educational dental accounts on the internet.  With 76K followers and growing, you’ll see why with their excellent images.

We start off talking about funny Thanksgiving stories.

We pivot into discussing cracks and fiber usage.

We lament the lack of cracked tooth education in dental school.

We talk about transilluminators.

We talk about using fibers as reinforcement.

We talk about fiber reinforced composite.

It’s always a good time when N8 the Gr8 joins us!

The Very Dental podcast is brought to you by our friends at Microcopy Dental. Neodiamonds from Microcopy Dental are very fine. I mean, all of them are awesome, but some of them are actually “very fine.” Specifically I mean the yellow striped ones they call “finishing” diamonds. I use Microcopy finishing diamonds on just about any patient that I might be spinning a bur on. Very fine finishers do the heavy lifting on composites so polishing is simple and easy. I use them to finish crown preps. I’ve been known to adjust Emax and Zirconia with them. Once you try them you’re going to find a ton of places to use them! And just like all of Microcopy’s burs, they’re sterile out of the package and single use means they’re sharp and efficient and can be disposed of at the end of the appointment! Go check them out at verydentalpodcast.com/veryfine (all one word)!

Cosmecore by Cosmedent is one of the most versatile materials in dentistry. I’ve come up with a lot more ways to use it than just core buildups. What new ways have you found to use it? Check it out at verydentalpodcast.com/cosmecore!

Al is joined by his best friend from dental school, Dr. Bart Schultz. They had a wide ranging conversation about dental school, early dental practice and how much patients can really understand about their treatment. Also, the jobs they had in dental school and other stuff.

My 11 year old son is in Invisalign with one of the orthodontists that I refer to. He’s doing REALLY well for an 11 year old boy. I see the case for his aligners all the time. With the orthodontist’s logo gloriously blazed across the lid. And then I thought to myself...aligners, splits and removable offers a brilliant way for people to remember your office and your brand!

Nate Lawson, DDS is a dental researcher who is the mastermind behind the @dentaltube instagram page which has been voted one of the top educational dental accounts on the internet.  With 76K followers and growing, you’ll see why with their excellent images.

We start off by talking about Curb your Enthusiasm and our favorite episodes.   Spoiler alert, if you’re watching the most current season airing now, episode 11.2 has some great scenes of Larry at the dentist.

We pivot into discussing the benefits of sandblasting in clinical and lab settings and white line formation in composites.

It’s always a good time when N8 the Gr8 joins us!

Thanks for listening and check out the Very Clinical Facebook page if you haven’t already.

Back when this podcast was called "The Dental Hacks" we used to include a segment in most shows that we called "Go Hack Yourself." It was a chance for us to talk about something we'd been using in our practice, listening to, reading, eating watching on Netflix or whatever caught our fancy at the time.

I'm bring in this tradition back starting with today's episode! I'm going to drop some spicy recommendations for the Very Dental Nation to try. Or not try. Either way is cool with me.

Recommendations in today's episode:

Dentistry is hard. We can only give so much of ourselves and we can only be compassionate to our patients for so long.

Dr. Brian McGue teaches sedation with 3D Dentists and at Implant Pathway and is acutely interested in helping dentists avoid burnout from compassion fatigue. So, what is compassion fatigue? Brian and Alan dig in to a topic that many of us relate to but probably haven't put into words yet.

This is the first full episode of the "We Are All Broken" podcast series. I'd love to hear from you if you find it helpful and worthwhile and perhaps some other topics you might be interested in hearing discussed!

The Very Dental podcast is also supported by our friends at Cosmedent! Cosmedent has just released a new composite that you’re going to want to try! It’s called “Renamel Bulkfill” and it’s here to simplify your posterior composite placement. It rivals the speed and efficiency of amalgam but it’s still packable, sculptable and polishes like a dream! You can cure up to 4mm increments, so it’s sure to increase the efficiency of working in the posterior. It’s radiopaque for easy identification on recalls, too! I’ll get a chance to try it clinically next week and I’ll give you my review with photos in the Very Dental Facebook group! Cosmedent is offering a killer 20% off deal for Very Dental Podcast Network listeners by using the coupon code “VDP21” at checkout on their website! So go check out Renamel Bulkfill at verydentalpodcast.com/bulkfill

Today we reach WAY back in the Very Clinical catalog! The gang is back.  Kevin, Mac, and Zach discuss posts and cores!

Highlights include:

- When do you choose to place a core, or a post?

- Do you remove all the previous restoration every time?  Most of the time?

How does Mac do a core?

- He mentioned like 5 products, Cosmecore, Absolute Dentin, Fuji 2, Equia Forte, and Miracle Mix.  I'm exhausted just typing all of those, so I pinned him down to one for technique.

- Cosmecore

- Mac bonds that in with a Self-Etching Primer Bonding Agent Prelude SE (technically he uses the generic KoolBond SE, but uses the Prelude branded dual-cure link)

How does Kevin do a core?

- He uses regular light cure composite sometimes (Kalore from GC), but usually uses Absolute Dentin from Parkell

- Bonds with Brush and Bond from Parkell

How does Kevin do a post?

- Dentatus Posts

- Buildup material cements the post

How does Mac do a post?

- Unicore Posts

- Buildup Material cements the post

Zach buildup and posts:

- Bulk EZ for buildups (autocure)

- Aforementioned KoolBond SE (generic of Prelude SE) to bond.

- Parapost XP

- FibreKleer Posts

- Vlock Posts (now Edenta Headmaster)
