I want to set the stage for today's throwback episode. It's May of 2020. Most dental offices are still closed down due to the Coronavirus pandemic. Many offices have been seeing emergency patients and were just waiting for the go ahead to open back up. What have we learned? What should we expect?

20/20 hindsight from September 2023 makes this episode a little dated. But it's a good reminder of what we didn't know. I hope you enjoy the time capsule.

Original show notes:

"Jason and Alan feature "what have we learned" from coronavirus and "what have we changed" due to it! We asked the amazing Dental Hacks Nation to tell us how the last few months has affected them and we got a lot of great answers. Dentistry is different now than it was a few months ago and from the sounds of things...it may even be better! (please note...some bad swears were used in this episode)."

  • Spear Education and the community that's been created around it. Martin describes it as a "family reunion."
  • Pandemic habits
  • The Habit Loop and its dental origin
  • Ritual vs. habit
  • How can you use this information to help you implement new things?

Dr. Marianna Evans is a dual specialty trained (perio/ortho) dentist who practices outside of Philadelphia! Marianna presented on the main stage at the 2022 Spear Summit on diagnosing and identifying the hypoplastic maxilla. Marianna's practice has a strong focus on treating the airway in children and they had a wide ranging conversation about airway, early orthodontic intervention and the "why" of airway developmental problems.

  • How can you help your orthodontist to embrace an airway paradigm shift?
  • Taking one case at a time with your interdisciplinary team
  • Focusing on evidence based medicine and dentistry
  • How CBCT changed how we think about orthodontics and airway
  • Much of the "airway literature" is in ENT journals, less in dental/ortho journals
  • How Marianna's network influenced the way she practices (in the same building as a pediatric sleep doctor!)
  • Marianna sees patients from age 0 to 75!
  • Why do we see airway problems now? Marianna has thoughts!
  • Knowledge problems vs. execution problems

In the very first “Off Topic” podcast Alan is joined by podcast veteran Dr. Melissa Zettler! Our plan was to talk about parenting but in pure “going wherever it goes” fashion, the conversation moved toward our mutual history at the University of Minnesota! Al and Melissa attended “the U” at the same time in the mid 90’s and they reminisce about jumping out of the lab window and stripping down to their skivvies in front of their classmates on the first day of gross anatomy!

Sorry not sorry, but this show starts off with a giant 10 minute rant about Saturday morning cartoons.  Listeners over 50 will delight in Kevin's re-telling of the early ages of 70s cartoons while Zach enjoyed the boys commercial cartoons of the 80's.

Dr. Alan Mead joins us to talk about electric handpieces and tries to sell Kevin and Zach on making the move.  You can't see it, you've gotta feel it...it's electric!

Alan tell us that he preps harder, faster, better, and stronger with the electric handpieces. He praises the torque, lightweight features, and variable speed.

Downsides include that they are more expensive to buy and maintain.

Kevin actually did a demo on an electric and disagreed on the lightweight feature, if anything he thought it was heavier and the difference in the way it cut didn't jive "in his hands".

Zach hasn't bothered trying electrics because he's cheap and has a good system with handpiece repairs on his Star handpieces.

Have you tried electric handpieces? Are they better? Worse?

Dr. Jason Smithson joins Alan this week to talk about specialization. Jason has taken a pretty interesting path to the career he currently has.

His first focus was going to be going through a fixed pros residency in the US with a plan to teach at a University in London along with having a cushy private practice gig to boot. Then things changed and Jason began to train in oral surgery, but once he realized the commitment he'd be making to more education, he switched plans again. He found himself in a General Practice Residency and actually stayed in that practice for upwards of 20 years!=== While he was here, he actually began training in a perio residency as well. Jason is a good person to discuss specializing with, but I think you'll find this conversation goes in directions you aren't expecting. We'll follow this conversation up with the second half next week!

Today's episode features Dr. Artie Volker talking composite veneers.

First we steal a joke and "remember some gals" with Sports Illustrated swimsuit models of the past.   Spoiler alert...we didn't remember that well.

How do you get involved with the artistic skill of resin bonding/veneers?

We talk bioclear resins vs. cosmedent

Chairside mock ups - one of the best ways to get case acceptance.

When to do composite vs porcelain

What composites work well for resin veneers?

What courses should you take?

If you want to discuss composite veneers, please join the Clinical Hacks Facebook Page and let us know what you think.

We kick off the episode by answering a REDDIT! from Lance in the way that they do on his podcast with Josh Austin called Working Interfences.    Did we do it justice?   Probably not.

Did you know Zach's Chiefs play Kevin's Browns this weekend?   Will they be on speaking terms after this weekend?  Whose side are you on?

We get into some fancy numbing technologies when we get around to talking clinical.     Lance uses the Wand.   It's really evident that Zach has never seen the Wand and didn't bother googling anything about the Wand prior to the show based on some of the odd questions.   My bad listeners.

Then we discuss Anutra.. kind of... none of the guests here use it, so we kind of speculated.. and Zach comes in hot with his DIY buffering method

We also talk topical anesthetics.. and some other anesthetic stuff.

Is any of this necessary?   Who knows.   Enjoy the listen,  check out the Clinical Hacks FB group and we'll catch you next week.

See ya!

This encore edition features the triumphant return of fan favorite, Dr. Jamison Spencer! Last episode we conquered sleep dentistry and airway problems. In today's episode, we tackle TMD!

A few high questions answered by Jamison in episode 205:

  • Should general dentists be treating TMD?
  • TMD patients are all NUTS (not understanding their symptoms). Most dentists are, too.
  • Why do we have so little evidence for the validity of TMD treatments?
  • What is the most common joint problem and simple treatment options
  • What's the difference between capsulitis and trismus and when should we treat it?
  • When should we give injections outside of the "deep dark hole" that we're used to?

It seems like everyone is talking about aerosols. The Coronavirus crisis has focused our attention on the possible spread of the virus in dental offices and specifically what we can do to reduce it and keep our patients and teams safer. We don’t have all the answers yet, but we do know that suction evacuation and rubber dam is a sure way to reduce dental aerosols. Our friends at Zirc have been producing some of the best products in these areas for a lot of years. If you aren’t familiar with the Zirc family of isolation products, they want you to take a look. You can get the Zirc “Protection Pack +” trial kit for only $9! The protection pack includes their amazing Mr. Thirsty One step (1 Pedo/small and 1 Medium/large) size. It’s the bite block high volume suction evacuator all in one that doesn’t need any complicated hardware. Just plug it directly into your high volume suction and you’re ready to go! The Protection Pack + also includes three “pink petals” which turns any saliva ejector into a hands free suction machine! And finally you get two of Zirc’s amazing Insti-Dam! The Insti-Dam is a nonlatex dental dam and frame all in one! Pre-punched for that emergency endo that shows up in your office tomorrow! Go check out the Protection Pack + at dentalhacks.com/protection and try one out for the low price of $9!

We know this has been an incredibly trying few weeks for really everyone, our dental friends and their patients. Many dental teams are just scratching their heads wondering, what should we be doing? Monitoring the regulations on a daily basis thinking, when are we going to get back to normal? What do we do with our appointments? You aren’t alone. There’s no book on this stuff, well not yet at least, but you know what? We have some advice for you, and it’s coming from our friends at YAPI. All of you that know of or use YAPI are likely familiar that one of their founders is Dr. Gina Dorfman, a practice owner herself. Dr. Gina and YAPI’s marketing team are going to help you stay productive and stay positive during this time. Whether you are closed altogether, treating emergency patients only, we’re going to be sharing a variety of different resources that will help you stay on top of this issue and how to stay engaged with your patients and make sure you don’t miss any opportunities that come your way. As COVID-19 survivor Tom Hanks once said, There’s no crying in baseball, YAPI says there’s no downtime in dentistry! Check the Hacks Facebook page regularly for your downtime checklists, webinars on teledentistry, shareable posts to keep you connected with your patients, advice on patient re-scheduling, and more.Go check them out at dentalhacks.com/YAPI.

Jason and Alan are joined again by Dr. Jamison Spencer. We've discussed sleep and sleep apnea with him, we've discussed TMD with him. Today we focus on why those two things go together! Some of the topics we discuss:

  • Why Jamison thinks dentists shouldn't limit their practice to sleep/TMD
  • Why TMD treatment is almost dead and sleep apnea is still the hot new thing
  • How new imaging techniques changed Jamison's paradigm
  • Do you need a cone beam to treat TMD and airway?
  • When would imaging change Jamison's treatment plan?
  • Why don't dental schools teach critical thinking and understanding the literature?
  • Jamison's requirements for a good scientific article
  • Why "doing stuff" is sometimes less effective than "kicking back and thinking about it" when you're treating sleep and TMD
  • How reps make all the difference in treating TMJ and sleep
  • The unintuitive thing that's going on when the sleep appliance causes pain
  • A new paradigm for treating sleep/TMD without insurance
  • When do you need to build a team to help you treat TMD/sleep?
  • Where is the medical community on sleep apnea?

Jason and Alan take questions from the Dental Hacks Nation. And the DHN comes through! In episode 228 we answer:

  • What are your favorite pizza toppings?
  • What was your most hilariously embarrassing moment?
  • Have you ever drilled into the wrong tooth? What did you say to the patient and how did you make it right?
  • What's for breakfast?
  • Why do you think it is important to go over to LMT LabDay during Mid-Winter?
  • How do you handle snow days at the office? Which one if you is the better snow shoveler?
  • What is the best gift to give to a patient of 20 years?
  • (Alan) What was the tipping point for your narcotic addiction and who or what made you reevaluate?
  • What’s the one piece of technology you don’t have but wish you did in your practice (or new practice in Jason’s case)?
  • Has All the new platforms (podcasts, FB pages, webinars etc) helped create more wonderful content and voices or created more wannabe experts masquerading as influencers and authorities?
  • Why is that people that graduated ages ago think they have the best advice for new grads?
  • How many bridges do you prep each year?

Thanks to all of the members of the Dental Hacks Nation for great questions! We'll be sure to do this again!

Dr. Darin Dichter is full time faculty at Spear Education. He's a prosthodontist, but practiced general dentistry for a lot of years before heading to UCLA for a pros residency! We talk about Spear Education, restorative dentistry and residency programs. We discussed:

  • How does Spear Education work? Online vs. campus? Seminar vs. workshop?
  • What was pros residency like for someone who had practiced dentistry for a lot of years?
  • How did Frank Spear react to Darin's choice about residency? How about Gregg Kinzer? And Bob Winter?
  • How does Darin feel about the "super GP" concept? (Darin doesn't like endo)
  • Are we "cutting bait" with teeth too early now that implants are so common? What about implant complications?
  • "Minimally destructive" vs. "minimally invasive" treatment planning. Trying to treatment plan yourself out of a case.
  • Wow does Spear talk about orthodontics a lot! Perhaps the least destructive treatment option out there.
  • Treatment planning by "thinking out loud"
  • The best way to break bad habits is not get them in the first place!
  • Why is photography important?
  • What's the best way to make sure you're always taking photographs?

Funny dentists. Two words that don't normally go together. Until now.

Episode 137 features a live round table of funny dentists. Dr. Mike DiTolla brought together a brain trust of dentists to help him work out material for the upcoming Dentsply/Sirona World in Las Vegas (September 13-16, 2017). You'll find it particularly interesting that the group was kicked out of our first podcasting location by the security guards at the Marriott in downtown Charlotte, NC...and we caught it on the recorder!

We talk about our own speaking styles, Gilbert Gottfried and the funniest dentists presenting today as well as what Mike's planning for the upcoming DS World! Please note...this has some adult content in it...maybe not appropriate if your kids are in the car!

You're about to witness some "Hack on Hack" crime! In our special year end wrap up Alan interviews Jason about dental startups. Jason has started 4 practices in his career and is just about to open his new practice, "Brush Up Dental" in Richmond, VA.

As you've come to expect from the Dental Hacks, Jason gives an honest account of the good, the bad and the ugly of his startup history. If you've ever thought about starting a practice from scratch or are pretty sure you don't want to...you should listen.

We really want to thank our listeners for all their support. If it weren't for you, we'd be two dentists sitting in the basement talking to ourselves. Instead, we're two dentists sitting in the basement talking at you!

The local pack. The 7 little balloons that show up on that Google Map when someone is looking for a dentist in your town. If you show up there, you're going to drive new patients to your practice. Period. Darren Shaw of Whitespark, a company that helps you improve your local search visibility, answers all kinds of questions in today's interview including:

  • What's the difference between organic search and local search?
  • What is a citation and why should we care?
  • What's the difference between a direct and indirect citation?
  • If you're doing a startup, at what point should you create/claim your local listings?
  • How should you handle local listings if you have multiple dentists in group?
  • How do you handle a local listing if you've changed jobs or moved your office?
  • Do citations have to be perfectly consistent? What kind of variations are O.K.?

On top of this, Darren talks about the importance of blogging. Which makes this blogger very happy.

Plus...if you use the code "dentalhacks15" you'll get a discounted rate on Whitespark products!

The Brain Trust (Dr. Matt Standridge, Dr. Frank Clayton, Dr. Tammy Bailey, Dr. Brent Young and Dr. Rich Rosenblatt) continue their discussion of how to handle "the unreasonables." Some things you'll hear in this episode:

  • Rich Rosenblatt hates bullies
  • Who do you need to contact at Delta to get an onlay covered?
  • How is the Waffle House different than Stuckey's?
  • Can truck stop dentistry be a legitimate business model?

The Brain Trust helped earn this episode it's first "explicit" warning. We editing out the cussing, but there was a fair amount of humor that was great but pretty blue. You've been warned!

CV4A9504Marketing is most effective when the content created is original and specific to the office. Todd Murray of Paper Napkin Marketing explains how dentists can leverage their assets to help make amazing marketing that fits their practice. Sure, using top notch video helps, but what about getting your team involved? How about spreading the word about how involved in the community your office is? Finally, Todd helps Alan understand that an entire marketing campaign needs to fit together. Blog posts are great, but how about making sure it fits with the message you want to send? This is another dental marketing "deep dive" that you won't want to miss!

Here's a sample of the kind of content that Paper Napkin can create for your practice!

The Brain Trust returns with an all star cast in episode 52. Dr. Frank Clayton, Dr. Tammy Bailey, Dr. Matt Standridge, Dr. Brent Young and Dr. Rich Rosenblatt all discuss handling the "unreasonables." How do you handle people with ridiculous expectations or terrible attitudes? When do you draw the line and "fire" these patients? This week's segment is hilarious but also very helpful. Don't miss it!

Linc is a dentist in the small town of Bundaberg in the province of Queensland in Australia. He's a self professed small town dentist who has had amazing success! In today's episode Linc talks about how his practice "out in the middle of nowhere" has become an amazing success! He's a marketing genius who's had luck with a lot of different modalities and he talks all about it with Alan and Brain Trust favorite Dr. Dawn Kulongowski!

In the second half of our interview with Dr. Armen Mirzayan we dig a little deeper into 3D imaging. Does it have value in evaluating past endodontic treatment? Does it replace all radiographs in the dental office? Also we talk about Armen's 10+ year stint as the official dentist of the LA Dodgers!

In episode 18 of the DentalHacks podcast the Brain Trust continues their discussion of patient "red flags." There were many true and hilarious suggestions on Facebook that we covered live during the recording! Jason and Alan joined Dr. Frank Clayton and newcomer to the Brain Trust Dr. Melissa Zettler in this enlightening discussion!

How often to we see deep decay that's completely asymptomatic? That's just one of the things that host Dr. Alan Mead talks to Dr. Robert Lowe about in today's interview. Dr. Lowe is a general dentist who practices in Charlotte, NC. On top of that, he teaches all over the U.S. and internationally. Dr. Lowe talks about new techniques in caries removal as well as using several new products to treat deep cavities without going right to root canal therapy.

The DentalHacks Brain Trust discussion for episode 15 begins a discussion on the worst and best purchases that they've made in dentistry. This episode of the Brain Trust features Dr. Thom Caspers of Seattle, WA, Dr. Tiffany Lee of Torrance, CA, Dr. Dawn Kulongowski of Holly, MI, Dr. Matthew Standridge of Eureka, KS and Dr. Tammy Bailey of Wausau, WI.

In today's episode, we continue our interview with Dr. Mike DiTolla. Mike shares some secrets with us about how cases are distributed at Glidewell Labs, how to removed (aka: cut off) zirconia crowns and he uses the word yttria correctly in a sentence several times. It turns out that the funniest guy in dental CE is also pretty darn smart. Who knew?

The DentalHacks Brain Trust discussion for episode 13 focuses on mistakes we made early on in our dental careers. Listen carefully to their suggestions about hiring! Pure gold. We introduce a new member of the Brain Trust in episode 13: Dr. Thom Caspers of Seattle, WA. Returning members of the Brain Trust are Dr. Frank Clayton of Suwanne, GA, Dr. Brent Young of Shelby, NC and Dr. Tiffany Lee of Torrance, CA.

Inspired by recent events, the first "DentalHacks Extra" episode has the Hacks taking on burnout. Every dentist from student to "soon to retire" experiences burn out. If you've ever got discouraged or tired of the profession, you should probably listen to this! Brain Trust members Dr. Frank Clayton and Dr. Brent Young discuss their own experiences with burn out and what some helpful solutions might be.

Dr. Tarun Agarwal is back in round two of his first (but probably not last) DentalHacks interview! First, we discuss dental implants and his teaching center, 3D Dentists in Raleigh, NC. Then he fills us in on how he attempts to balance teaching, clinical dentistry, being a father and managing his weight. If you liked the first half of Tarun's interview, you're going to love episode 12!

The DentalHacks Brain Trust discussion for episode 12 is a continuation of our answer to a  listener's question. We all have our typical daily routines, but since we live in our own little bubble, the listener wondered what everyone else was doing. Today you'll hear about Dr. Frank Clayton's shopping habit, Dr. Dawn Kulongowski's childbirth experience and Dr. Tammy Bailey's $8 nap!

Has your office suffered from poor online reviews? The internet has allowed everyone to have their say about any business. Great reviews let the world know that your office is the one they should choose, but what about the bad ones? Many sites allow the reviewer to be anonymous, so what is a dentist to do?

Dr. Doug Wolff returns in part two of the DentalHacks interview to discuss exactly this. What are the legal ramifications of poor reviews? Can you do anything about them? Dr. Frank Clayton sits in with Jason and Alan for an interesting talk.

Then, you can join the DentalHacks Brain Trust to discuss what they do when a patient won't let you lean the chair back, informed consents for all kinds of procedures (even dentures!) and how to help patients when they've had recent dental work done that's not very good.

This week’s Brain Trust segment featuers Dr. Brent Young of Shelby, NC, Dr. Frank Clayton of Suwanne, GA, Dr. Tammy Bailey of Wausau, WI and Dr. Matt Standridge of Eureka, KS. The Brain Trust delves into the treating of the high caries patient as well as how to not take things personally when patients choose to put off or never complete treatment. They ask the question “why do my patients take the specialist’s word and not necessarily mine?” Funniest Brain Trust segment yet? We think, "yes!"
