Clinically, Chris took an implant residency post dental school in implant restorations and he talks about how implant restorations sparked his speaking career which sparked his entire career in the spotlight.

All are based on being a pain in the ass for the client.

  1. The $x/month credit card swipe “subscription.”
    1. “New Paper”
    2. Spotify and other subscriptions, if you aren’t careful
    3. Based on being difficult to cancel. Or at least multiple steps. Often inexpensive enough that you won’t bother.
  2. Verizon’s “you can only cancel by calling this number”
  3. Automatic renewal by credit card of ANYTHING
  4. Medicine’s “automatic premium payment” and “never discussing money before a procedure” technique.

Rob is a proud “restorative dentist” by choice and has described his approach as “from the gums up.” How did he decide what he wanted to include in his practice? Why did he decide to leave a lot of “specialty” procedures out? Some topics we covered:

Graham’s story: He loved what he saw from the Eastern European scope gurus (Dr. Maxim Belograd and Dr. Nazariy Mykhaylyuk) and found that a full day of restorative dentistry was getting hard on his neck!

You know what sucks?  Well lots of things I guess, but in dentistry one of the many things that sucks is when you put in a 2nd molar crown or MO filling only to see it on recall and the patient is complaining they are packing food around it.   Why does this happen?   

You know what sucks worse?  When you re-do said restoration for free only to see it happen AGAIN.

Why does this happen?

What can you do about it?

Kevin offers some solutions orthodontically.

Zach gives some bad advice out of frustration for incidents where this has happened to him. “Splint early and splint often”.

Have you ever heard of "Getting Things Done?" It was a book/system to help people be more productive. Al was affected by this back in the blogging days (the late oughts) and found an interesting practitioner by the name of Merlin Mann. Merlin's blog "43 Folders" was on my reading list for years between 2008-2012 or so. He's an interesting thinker and he coined a phrase that stuck with me. That phrase was "ubiquitous capture."

Alan is joined by Dr. Gina Dorfman and Zak Allmand to talk about dentists with side gigs! 

Gina created YAPI which allows dentists to be paperless, allows offices to communicate within the offices as well as with patients (texting, reminders, etc.), get patient reviews as well as online appointments! YAPI does everything you wish you practice management software would do!

Zak started Apex Payment Solutions, a company that helps dentists take credit card payments. According to Zak, most credit card payment companies are "shady af." Apex helps dentists get better deals on taking payments! 

The conversation was really interesting because we dove into side gigs for dentists:

  • A great dental side gig comes from solving a problem that dentists are having (paperless solutions, credit card payment rip offs, no podcast that I wanted to listen to)
  • Zak is a Viking, hates the dental tax
  • The side gig is often "the next adventure"
  • Idea people need help executing their ideas
  • Side gigs are often not tied to a specific place, unlike practicing dentistry
  • Abundance = not caring what the patient does after you've explained the options
  • The dream isn't always to build something big enough to sell out. We'll always need the next adventure.
  • Should new dentists be looking for side gigs? (spoiler: no)

  • Did fear of COVID change Al's behavior?
  • How much dental disease is preventable by behavior change (home care, diet, smoking, etc.)?

Dr. Mark Whitefield joins Alan for a rousing discussion of how implants can change everything for the general dentist. Mark is a fourth generation dentist from Nashville who admits to being obsessed with dental implants!

  • Mark talks about being accredited in both the AACD and AAID and how that's made him a better dentist.
  • Mark graduated in the mid 90's during the big cosmetic boom but switched his focus to dental implants in the early 2000's.
  • What I noticed about Mark is that his mindset is that dental implants can be for anyone. Not just high end/wealthy patients.

Al is joined by long time listener favorites Dr. Frank Clayton and Dr. Melissa Zettler for a talk about regrets. Between the three of them they've had 72+ years of experiences of dentists and practice owners. Al asks the question: what do you wish you had done better? What regrets do you have? And man did this Group Function come through! 

  • Start saving EARLY! (no, even earlier)
  • Avoid buying stuff on payments
  • Use your student loan money wisely
  • Buy used cars
  • Not letting employees go sooner
  • Take more time off...being away from the office is NOT the same as "losing money." Not even close.
  • Take CE, but don't be in a rush.
  • Choose CE early that can pay off over a longer career.
  • Get to a point where you don't care if the patient follows through with care.
  • Lay the patient back far enough. The patient is only there for an hour. You're there ever day.
  • Orthodontics is a great choice for a lot more patients than dental school ever taught you.
  • Make connections with honest colleagues...find people to give honest feedback

The first episode of the new podcast (The Very Dental podcast) starts the same way that the old podcast did! With Dr. Lance Timmerman! 

Lance once again graces us with his presence for Al to explain where the podcast is heading.

The two of them hop in a time machine to the day after the graduated from dental school to offer their past selves some ideas that might make their lives a bit easier. Some highlights:

  • try a lot of different ways of practice
  • get reps
  • you don't have to do everything if you don't want to
  • It's hard to dabble in specialty procedures...so be careful before jumping in
  • Magnficiation...just do it
  • You probably can't afford the stuff you think you deserve yet

We open up the podcast with a fitting tribute to DMX (maybe only marginally fitting). As an aside Zach would also like to use this space since we won't talk about it on the podcast to also give a tribute to the passing of Shock G from Digital Underground, as the Sex Packets album was his first "Parental Advisory" cassette as a youth.

After a cringe-worthy DMX vs. Shakespeare discussion we immediately get into our clinical topic: Routines

How do you settle on the way you do a procedure?   I suspect 10 different dentists would do the same procedure 10 different ways... why?  And how do you decide on what you do?

Well that's what we talk about. Crowns, operative..and much much more.

3 dentists, 3 completely different systems. Fully digital, semi-digital, analog. There's room for everyone.

I was introduced to Dr. Eric Herbranson in 2011 at the Academy of Microscope Enhanced Dentistry where he talked about the 3D Tooth Atlas he had created. Both are influential endodontists who have controversial ideas about how we've been treating teeth over the years.

Today's episode features a journey from patient based outcomes to case selection, access openings to how we know what we know. There's a fair amount of hero worship here I've been a huge fan of these guys for quite some time. And a bonus...the one and only Dr. David Clark of Bioclear sits down at the end and talks endo!
