• Kevin’s #16 Crown Prep: Kevin recounts the dread of seeing a crown prep on a third molar (#16) at 8:00 AM. Despite the technical difficulty and the "Is this a joke?" feeling, it ended up being successful with a rubber dam.

• Zach's "Self-Extracting" Molars: Zach shares a cautionary tale from his early career working with his father. He attempted to crown a second molar that was tilted so far distally it was practically trying to extract itself. The result? A lost contact, a lost crown, and eventually, a lost patient.

• The Bioclear Pivot: Alan discusses a recent case where he planned a crown on #15 but found decay so deep it was near the crest. He pivoted to a side-by-side Bioclear composite, which was harder and more time-consuming but ultimately the right clinical choice.

Categorizing the "Resurrected" Patient

The main theme of the episode focuses on patients who reappear after years—sometimes a decade—away from the chair. Alan and the hosts break these down into tiers:

1. The S-Tier (The Best Case)

These are patients who didn't leave because of a conflict; they simply "had a gap in life."

• Examples: Moving away and moving back, going through a divorce, or the "COVID-12" (people who stopped coming in 2020 and are just now emerging).

• The Dynamic: They still feel like "your" patients. They often have a high follow-through rate because they are relieved to be back in a familiar, trusted environment.

2. The A-Tier (The "Grass Wasn't Greener")

Patients who left for a specific reason—usually insurance—and realized that "a dentist is a dentist" isn't actually true.

• The Insurance Trap: Many leave because the office went out-of-network, only to find that the "in-network" experience lacked the technology or relationship they valued.

• The Result: When they return, they often have a newfound appreciation for the higher level of service and clinical care provided.

3. The Lower Tiers (The Friction Cases)

• The Billing "B-Tier": Patients who disappeared because they owed money. They usually only reappear when they have an emergency and are suddenly willing to pay their old balance to get out of pain.

• The Personality "C-Tier": Patients who left after a confrontation or a bad review.

Professional Growth & "Hard Nos"

Alan reflects on how his clinical confidence has changed over the years. When a patient returns after 10 years, they aren't seeing the same dentist; they are seeing a version of Alan that is more skilled, has better tools (like new Bioclear matrices), and more experience.

When is a "Resurrection" a "Hard No"?

• Abuse of Staff: Rudeness, yelling, or cussing out the team.

• Chronic No-Shows: People who are "constitutionally incapable" of making it to an appointment.

• The Dismissed: If a formal dismissal letter was sent, the door remains closed regardless of the patient's willingness to pay.

Despite initial skepticism from colleagues about "shitty crowns," a half-day training session with Dr. Bob Conrad ignited his passion for the technology, leading him to fully embrace it, become an owner-doctor, an early adopter of the Omnicam, a renowned speaker, and the founder of the popular Keep CERECing Facebook group in 2016. He concludes by announcing the upcoming Third Annual Keep CAD Camming meeting in Austin, October 15th-17th, 2026, which will expand beyond CEREC to cover a wider range of CAD/CAM and digital dentistry topics.

For the early career dentist (Years 1–7), Frank advises working for a couple of years, then investing heavily in CE to establish a clinical niche (like his focus on form, function, and aesthetics), and, most critically, living well below one's means while investing consistently with a dental-specific financial planner—advice that Kevin reinforces by telling listeners to "gamble on yourself." During the middle-game grind (Years 10–25), Frank recommends shedding unhappy or unprofitable procedures to embrace the "boring" but profitable, repetitive work, while utilizing advanced tax-sheltering strategies like Cash Balance Plans; Kevin shares that he reinvigorated his middle career by learning implants, which provided financial and time freedom. Finally, addressing the last third of a career (Years 25+), Frank stresses the importance of viewing the practice as a valuable asset and advises starting the exit planning process five years out, focusing on selling at the top—by knowing the office's maximum production capacity (the Law of Diminishing Returns)—rather than limping to the finish line. In retirement, Frank finds liberation in being "just Frank," free from the burden of daily high-stakes decisions, and encourages other dentists to find a new focus rather than fearing the loss of the "doctor" identity.

• Profound Anesthesia: Mead's first step is always to achieve profound anesthesia, utilizing inferior alveolar nerve blocks for lower molars and buccal infiltrations (usually with Septocaine) for other teeth. He heavily relies on the  Septodont Paroject pdl syringe for supplemental tissue anesthesia around the entire tooth, often using it to avoid packing cord.

• Pre-Op Digital Scan: He scans the arch using his Medit i700 before prepping the tooth. This pre-op scan is used to register the final prep scan, ensuring a stable and accurate bite registration, which he praises as a massive efficiency improvement over traditional PVS impressions.

• Prep Sequence: His prep sequence is intentionally designed for efficiency:

  1. Break Interproximal Contacts First: He uses a Microcopy 0512C inverted pear bur for gross contact breaking, followed by a Microcopy 33140.10vf ("skinny") fine-tipped diamond for careful separation at the gingival.

  2. Occlusal Reduction: He uses the Microcopy 2137C which large, barrel-shaped coarse bur in an electric handpiece for quick, proper occlusal reduction.

  3. Core Build-up: After removing any old restoration or decay, he prefers Cosmecore from Cosmedent for build-ups, sometimes blocking out with Fuji IX from GC. He utilizes an old-school metal matrix band with Bioclear Diamond Wedges for isolation during the build-up.

• Margin Refinement and Tissue Management: He finishes margins under high magnification (microscope) and prefers a very fine diamond bur (Microcopy 3116.8vf) instead of traditional polishing cups for a smooth finish. Mead emphasizes smoothing the adjacent teeth with a Sof-Lex disc to ensure the lab can create an ideal, broad interproximal contact.

• Retraction: Mead overwhelmingly prefers Traxodent from Premier in the unit dose syringe tips for quick and clean tissue retraction, stating it generally works better for digital scanning than retraction cord.

• Temporization: While his assistant fabricates the temporary using a pre-made matrix, Mead remotes into his computer to mark the margins on the digital scan. He uses Clinician's Choice Template Ultra Quick Matrix Material and Mirror Image temporary material from Cosmedent, which he quickly refines with a Sof-Lex disc. He cements the temp with Dentotemp by Itena provisional cement for its strong retention.

He suspects much of the initial clin-check setup is AI-driven, with human checks afterward.

• Design & Manufacturing: AI is already used for the automated design and quick file return for appliances like night guards by companies like SprintRay, as well as for crown design in some printing workflows.

• Radiographic Diagnosis: The hosts discuss companies that use AI to read and assist in diagnosing findings on X-rays. A key discussion point is how these companies, like DSOs (Dental Service Organizations) such as Heartland or Aspen, acquire the vast amounts of patient data needed to train their models. In this throwback episode Zach and Kevin feature Dr. Bob Convissar, a pioneer in laser dentistry with over 32 years of experience. He's a prolific author, lecturer, and practitioner who began his career after graduating from NYU in 1980 and acquiring a unique, century-old dental practice in New York City

The main topic is the ABC's of Laser Dentistry, starting with a critical distinction between true lasers (like CO2​ and Erbium) and devices often called lasers but primarily used as hot glass tips (like diodes). Bob explains that diodes, which can operate at extremely high temperatures (750∘C to 1500∘C), work thermally, creating a burn, while CO2​ and Erbium lasers are absorbed by the water in soft tissue and vaporize it at 100∘C.

He emphasizes that the CO2​ laser is the best option for general dentists doing soft tissue work, as it creates a much smaller zone of necrosis (thermal damage) than ElectroSurge or diode lasers, leading to better healing. He also offers a crucial financial perspective, arguing that while a CO2​ laser may have a higher initial purchase price (around $30,000), its lack of expensive disposable tips makes it a more cost-effective long-term investment than a cheaper diode laser (like a Picasso) with high operating expenses (potentially $18,000 in tips per year).

For a multi-faceted approach, he recommends dentists invest in a CO2​ laser, a modern air abrasion unit, and a Photobiomodulation (PBM) laser (around $2,000 to $3,000). PBM lasers are purely palliative and can be used for pain management, like treating TMJ issues or for analgesia on deciduous teeth, and for promoting healing after invasive procedures.

Finally, Bob stresses that the most crucial factor in laser dentistry is training, advising listeners that any course focusing on only one manufacturer or wavelength is likely a sales pitch, not a comprehensive educational seminar. • Al changed his mind about being fat. NOW is the time to pay attention to your health. It's way better not being fat.
• Kevin changed his mind about diet and exercise. He moves his body every day.
• Al changed his mind about waiting. Don't wait.
• Zach changed his mind about paying attention. Zach doesn't pay attention to current events now (outside of sports) and his mental health is so much better for it. What can you control?
• Kevin changed his mind about stuff vs. experiences. Experiences are better than getting stuff.

• Al has changed his mind about creating urgency. Having less hours available makes patients less likely to fail appointments.
• Kevin changed his mind about EFDA's. EFDA's make Kevin's office work. "Other people can be good with arts and crafts."
• Kevin changed his mind about offering the best treatment. "Let your patients say 'yes' to your best care."
