Jason and Alan are joined by two dental social media rock stars, Dr. David Rice of IgniteDDS and Irene Iancu (aka: toothlife.irene) of the Tooth or Dare podcast!

David knows young dentists. He's an educator and coach and spends a lot of his time helping new dentists find their way after dental school. His message is a lot more hopeful than what you hear from some people and his reach is really huge! David also spent some time talking about 3M's new Xerostomia Relief Spray. Dentistry hasn't done a great job for our patients with dry mouth and 3M has made a product that can really make a difference for them!

Irene Iancu is a hygienist, educator, social media maven and entrepreneur! She's got a great podcast called "Tooth or Dare" and she'll be podcasting live at the Voices of Dentistry with her new co-host. She's fascinating on a lot of levels, but we spent a lot of time explaining how she, as a hygienist, is opening a dental practice! If you've ever wondered how a dental practice owned by a "nondentist" can work, this is really interesting stuff! Apparently delays in dental office building affect everyone!

Teresa Duncan joins Alan to talk about the recently legal action taken by the American Dental Association against Delta Dental. Teresa's company is Odyssey Management where she used to be primarily a consultant. Now she spends most of her time educating dental teams about all things dental insurance!

This timely topic is a "must hear" for all dentists and their teams. Teresa features an in depth discussion about the lawsuit on her own podcast, "Nobody Told Me That with Teresa Duncan."

  • So what does this lawsuit mean for me?
  • The three main points of the lawsuit:
    • market allocation conspiracy
    • price fixing conspiracy
    • revenue restriction conspiracy
  • monopsony is a market structure in which a single buyer substantially controls the market as the major purchaser of goods and services offered by many would-be sellers.
  • The CEO pay gap between Delta and other "for profit" companies
  • Delta Dental "reserves"
  • Claims processors = data collection for insurance companies
  • Speculation on what might happen with this lawsuit
  • Al pitches the ADA on a podcast

Jason and Alan had the chance to interview a bunch of superstars at the Greater New York Dental Meeting in NYC! 3M let us use their amazing booth to speak with some really interesting folks including Dr. John Weston and Dr. Artie Volker!

John is a frequent speaker on cosmetic and restorative dentistry and the Hacks talked with him about the ins and outs of seating indirect restorations!

  • The only cement in John's office: Unicem 2
  • Treating Zirconia...how do you clean (Zirconia Oxide...Ivoclean, Zirclean)
  • Unicem 2 cementation technique (one second, tack cure)
  • Multiple unit cementation (light cure)
  • QwikStrip
  • Do you really even need Ivoclean? Could alcohol or hypochlorite work? (3M says yes!)
  • Try on protocols
  • High Translucency zirconia vs. lithium disilicate
  • Anesthesia for crown cementation (almost never!)
    • gluma for the unanesthetized  patient

Artie Volker has been on the show a bunch of times and the native New Yorker doesn't disappoint! Jason and Alan discussed composites and the Bioclear method with him, with the one and only Dr. David Clark jumping in a couple times to steal the show! (he just couldn't resist!)

  • Artie took a train to see us at the GNYDM!
  • The scourge of "black triangles!"
  • Using Bioclear to finish orthodontics/Invisalign
  • The Bioclear road show
  • Patient centered vs. dentist centered care and "Show me the x-ray!"

Jason and Alan had the privilege to speak with Dr. Rebecca Bockow, a dual trained orthodontist/periodontist and cutting edge advocate for understanding airway. Rebecca has been teaching at Spear Education for a couple of years and will be one of the instructors in the newly revamped "interdisciplinary treatment planning" course that Spear is putting together. She's also been featured on our "Queens of the Airway" episodes as well. The Dental Hacks had a wide ranging discussion with Rebecca which included:

  • How does a dentist decide on a dual specialty and why orthodontics/periodontics? (spoiler: she almost ended up as a prosthodontist!)
  • Expansion before expansion was cool
  • The Airway "Ah HA" moment
  • How teaching is a type of marketing for a reknowned specialist like Rebecca
  • The paradigm shift to treating younger patients orthodontically
  • Pediatric airway screening with a one page questionnaire
  • What in the whole world needs to change to better address airway issues
  • How can regular dentists understand and evaluate the literature about airway
  • How to rehabilitate a patient with excessive wear/gummy smile and more
  • Looking at how the malocclusion/skeletal development came to be
  • What's Rebecca's "elevator pitch" about how to incorporate airway into your practice
  • How do gummy smiles and airway relate to each other?
  • How to open the palatal suture once you're an adult (TAD expanders vs. Hyrax expanders)
  • Surgery + orthodontics = airway magic
  • What % of people breathe through their nose?
  • How do we evaluate tongue tie?

Becca is a genius and if you get a chance to see her teach, DO NOT MISS IT!

Jason and Alan speak with Dr. Mitch Ellingson about running a business. Mitch is in charge of Spear Practice Solutions, the business/consulting arm of Spear Education. Most dentists are going to run into this quandry...how can you be a great clinician and run a great business. Mitch has some thoughts. A lot of them as it turns out! Some of what we discussed:

  • The difference between a consultant and a business system
  • Can your life look like a Newport cigarette commercial?
  • What does "big data" look like with Spear Practice Solutions?
  • Case acceptance...it's not like you think (spoiler: it's about recare efficiency)
  • What is leadership really? How can Spear Practice Solutions help with this?
  • Are consultants really just brought in to fire someone?

Our clinical outcomes often depend on light cured materials. Dentists take for granted that their lights are delivering the right amount of energy, but how can you know? If you have more than one light, are they delivering the same amount? How can you make sure you aren't undercuring your light activated materials while minimizing the heat from your curing light? Alan and Jason are joined by Dr. Chris Salierno and Chris Felix of Bluelight Analytics to talk about curing lights! Chris is the chief scientist at Bluelight Analytics which is a company that helps dentists measure the quality and intensity of the curing lights in dental offices.

Our friends at Bluelight are giving away one of their checkUP devices with a 1 year subscription on the Dental Hacks Nation! All you have to do is like and comment on our post there and you're entered to win!

Jason and Alan had the privilege of sitting down with the one and only Dr. Frank Spear. We had an hour during the Spear Summit to pick up where we left off from episode 235 and it was very interesting. Frank opened the meeting with a talk that discussed the stories that dentists tell each other about where the profession is going and then presented some data that suggests a lot of these stories aren't exactly true. Some of the things we discussed:

  • Why do insurance companies cover thing so differently from state to state?
  • The beauty of practicing in groups. But not necessarily a DSO.
  • Could someone become Frank Spear in the way that Frank did considering student debt as it is now?
  • How did beautiful temporaries make a difference in Frank's career?
  • Why does Frank feel responsible to help the narrative that dentists tell each other?
  • What's the data the Frank would really like to see?
  • Why is it so impossible to compare dental businesses and income across the country? How can you compare apples to apples?
  • Take home message: things are better than dentists are telling themselves!
  • What is dental school like when your dad is Frank Spear?
  • How could dental school teach things with more context?
  • Are there useful parallels between dental and medical education?
  • How will Frank's teaching schedule be changing in the future?
  • How is the curriculum at Spear Education changing?
  • What kind of "big numbers" has Frank been impressed with lately? (spoiler: it has to do with Spear Practice Solutions)

Jason and Alan are joined again by Dr. Jamison Spencer. We've discussed sleep and sleep apnea with him, we've discussed TMD with him. Today we focus on why those two things go together! Some of the topics we discuss:

  • Why Jamison thinks dentists shouldn't limit their practice to sleep/TMD
  • Why TMD treatment is almost dead and sleep apnea is still the hot new thing
  • How new imaging techniques changed Jamison's paradigm
  • Do you need a cone beam to treat TMD and airway?
  • When would imaging change Jamison's treatment plan?
  • Why don't dental schools teach critical thinking and understanding the literature?
  • Jamison's requirements for a good scientific article
  • Why "doing stuff" is sometimes less effective than "kicking back and thinking about it" when you're treating sleep and TMD
  • How reps make all the difference in treating TMJ and sleep
  • The unintuitive thing that's going on when the sleep appliance causes pain
  • A new paradigm for treating sleep/TMD without insurance
  • When do you need to build a team to help you treat TMD/sleep?
  • Where is the medical community on sleep apnea?

"Nothing others do is because of you."

--Miguel Ruiz, "The Four Agreements"

But here's the deal. Nothing that YOU do is because of what others have done. What you do is your choice.

Man. That sucks. But it's true. In today's episode I talk about how what happens to me isn't necessarily in my control but how I choose to react to it is. Every single time. It's a tough pill to swallow, but I think this will lead to significant peace in my life and I hope yours, too.

Jason and Alan are joined by prosthodontist Dr. Doug Benting to talk about treating the edentulous arch! A lot of dentists have very little experience with dentures and edentulous arches and for this reason, many (most?) dentists don't like dentures! Doug is here to help!

  • Is a high school kid that starts working in a dental lab destined to be a prosthodontist?
  • Is "all on x" all it's cracked up to be?
  • How do you know when someone's dentition is "terminal?"
  • Are we too quick to take teeth out?
  • Dealing with the psychology of people who are probably going to lose their teeth
  • New and interesting ways to treat edentulous arches with implants
  • Keeping teeth even though you know they're eventually going to be lost?
  • What's up with digital dentures? Printing dentures vs. milling
  • Instagram vs. the fundamentals...why learn dentures?

Alan has talked about (bragged about?) The Francis B. Vedder Society on the show for years. He's been a member for 20 years or so and it's been very influential in his career. Some of the speakers Alan has seen in the last 20 years or so include Dr. Frank Spear, Dr. John Kois, Dr. Jeff Rouse, Dr. Jeff Okeson, Dr. Stephen Chu, Dr. Pat Allen, Dr. John Burgess, Dr. Bernard Touati and Dr. Bob Winter to name a few. It's been a literal "who's who" in dentistry lecturing to a small group of colleagues in a laid back setting.

Last weekend was the 2019 Fall meeting and the society celebrated 60 years of continuing education and fellowship. Alan recorded a "live podcast" at the event where many of the members of the Vedder society shared some funny and emotional stories of what the society has meant to them. We hope you enjoy this unconventional episode and maybe you'll consider a trip to northern Michigan next fall (September 24-27th, 2020)!

The two of them are launching a new podcast called "The Peaceful Practice Podcast." Alan and Dawn have been friends for over a decade. There is a teaser episode on your favorite podcasting software and the first episodes drop October 7! Go subscribe now and don't miss a single episode!

Your office has to do a LOT of stuff. You’ve got to take in and organize a ton of patient information and make sure it shows up in the right place at the right time. You’ve got to be able to communicate throughout your office at any time, yet you need to do it discreetly. As much as you wish you didn’t have to...you need to remind your patients of their appointments and try to avoid broken and failed appointments. You have to reactivate the patients that have fallen through the recall cracks. And in 2019...you have to be able to ask your loyal patients for reviews in the right places like Google, Facebook and Yelp. Our friends at YAPI have solutions for all of this and more! And they can do it paperlessly! Check out all that YAPI can do for you at dentalhacks.com/YAPI. It’s a funny name, but it’s serious software!

Have you ever wished you could go to dental school as a dentist? Wouldn't it be great to have the experience and perspective that you have now when they were trying to teach you the basics? Jason and Alan talk to Dr. Gregg Spear of Spear Education about this very concept. When we were being taught about occlusion in dental school we didn't really understand why we were learning this nor how it affects our treatment. Jason and Alan were at Spear Education taking the phenomenal Occlusion Workshop and talked with Gregg about how Spear kind of ends up being "dental school for dentists." Some topics that we cover:

  • Dentists as introverts
  • Becoming a teacher
  • Spear Summit
  • How Spear Education instruction has changed over time
  • The "red pill" aspects of airway and joint issues
  • Work-life balance
  • magnification and dental microscopes
  • light sources
  • Incorporating digital workflow at Spear
  • Saving teeth vs. placing implants
  • What would you tell a first year dental student? What would you tell a 4th year student?

Welcome to season 1, episode 15 of the Clinical Hacks Podcast

This week we gave Mac Jones the week off to build a patio table and instead we talk about sealants.

Zach Meiners, Kevin Fryer, and Alan Mead have one thing in common, we all pretty much dislike sealants!    What?  Aren't they preventing decay?     Enjoy what's likely an unpopular take on sealants.

Highlights include:

- Do sealants prevent decay?

- Can sealants "cause" decay?

- What conditions should be met for sealants to be effective?

- Is adding sealants into your hygiene program a revenue booster?

- When did 3 experienced clinicians turn sour on sealants?

- How does a microscope user view the sealant restoration?

- Is the medium quality sealant the worst restoration?

- Glass Ionomer sealants, do they have a place?   Fuji Triage

Everyone talks about delegation. It sounds great, but it's not as easy to do as you would think. Dr. Gina Dorfman is a person who gets a lot of things done. We talk about a lot of things in this short podcast. Here are a few of them:

  • Perfectionism isn't a good thing. On the whole, it's a bad thing. It keeps you from starting because you might not make it perfect.
  • Done is almost always better than perfect.
  • Al gets therapeutic about podcast production
  • Do things that you like to do the most. Not only that you're good at, but you ENJOY doing.
  • Finding the right people to delegate to is worth it
  • Why being on Facebook isn't always a waste of time
  • Finding associates on the beach in Hawaii
  • Training is the most important part of delegation
  • If everyone always disappoints you, it's probably you
  • Can your office run without you?
  • Are you a micromanager?

Jason and Alan sat down to talk with Dr. Darin Dichter from Spear Education. They were taking the 3-day Occlusion Workshop and Darin was one of the instructors. This wide ranging discussion is spectacular and covers:

  • Occlusion philosophies
  • Dental School
  • Continuing education and the struggle to implement ("getting reps")
  • Student loan debt
  • Not all dentists are clinicians!
  • Critical thinking and understanding the literature
  • Being a nontraditional student and perspective outside of school
  • What is a terminal dentition, when do you know how to treat it and what's the "Treating the Terminal Dentition" course all about?

Dr. Alan Mead talks about his experiences with difficult patients and offers a few tips.

  • Find the red flags before you treat
  • Patients can’t/don’t/won’t understand treatment...what can you do about it
  • Create a relationship and find out when it’s less about your or your treatment and more about their life.

These tips aren't bulletproof, but they'll get you out of a lot of "difficult patient" situations!

Welcome to season 1, episode 12 of the Clinical Hacks Podcast

The gang is back.  Kevin, Mac, and Zach discuss posts and cores

Highlights include:

- When do you choose to place a core, or a post?

- Do you remove all the previous restoration every time?  Most of the time?

How does Mac do a core?

- He mentioned like 5 products, Cosmecore, Absolute Dentin, Fuji 2, Equia Forte, and Miracle Mix.  I'm exhausted just typing all of those, so I pinned him down to one for technique.

- Cosmecore

- Mac bonds that in with a Self-Etching Primer Bonding Agent Prelude SE (technically he uses the generic KoolBond SE, but uses the Prelude branded dual-cure link)

How does Kevin do a core?

- He uses regular light cure composite sometimes (Kalore from GC), but usually uses Absolute Dentin from Parkell

- Bonds with Brush and Bond from Parkell

How does Kevin do a post?

- Dentatus Posts

- Buildup material cements the post

How does Mac do a post?

- Unicore Posts

- Buildup Material cements the post

Zach buildup and posts:

- Bulk EZ for buildups (autocure)

- Aforementioned KoolBond SE (generic of Prelude SE) to bond.

- Parapost XP

- FibreKleer Posts

- Vlock Posts (now Edenta Headmaster)

Jason and Alan are joined again by the legendary Dr. Mike DiTolla! In this wide ranging conversation we talk about all kinds of stuff. Mike claims to be a charter member of "Underpreppers Anonymous." He's seen a lot of tooth preps in the world's largest dental lab over the years and the bottom line is this: dentists routinely underprepare teeth. Particularly molars. One of Mike's goals in life is to help dentists do it right. If you prep crowns, this is a valuable discussion about preparation techniques, intraoral scanners and tools for more ideal reduction. We also discussed the future of hands on dental CE and bigger dental meetings. Finally, we talk about what's happening in the world of full contour zirconia (including bridges) since Mike was on the ground floor at Glidewell when Bruxzir was being developed!

Jason and Alan are joined by DSO luminary Vin Cardillo of Maeva Dental Advisors! Vin has been an integral part of the growing trend of DSOs spreading throughout the United States and he understand the industry from top to bottom! We discuss the history of DSOs (and where Vin fits in there), changes and growth and perhaps more importantly what kind of dentist makes a good fit with the DSO model. You might be surprised to know that perhaps most dentists aren't cut out to be leaders of this kind or organization and quite a few that get into it are really struggling. Vin gave us some amazing inside information and he's a true resource for any dentist considering multiple practice ownership! His company has recently rebranded from Dental Management Innovations to Maeva Dental Advisors, so be sure to look at both links for the most up to date contact information!

"You get the rough with the smooth."

--Dr. Jason Smithson on having a specialty

Alan and Jason continue their conversation about specialization. This episode is where we get to the good stuff where we really dig into the pros and cons of specialization and how generalists that long to be known for a particular kind of work might have it wrong! Jason debunks some of Alan's preconceived notions about how having a referral base makes a patient choosing treatment almost automatic. And Alan is mildly crushed.

Also we discuss the excruciating problem of having a 20+ year old practice/patient base and the ups and downs of having a "history" with people. Finally, we open the door to a potential next podcast to talk about an ideal way to do group practice.

This is a really good one. Don't miss a minute!
