Alan delves into a thought-provoking ethical dilemma presented in the Very Clinical Facebook group regarding pain management for a patient in recovery.

  • Pain Management Dilemma: A detailed discussion of a Facebook post in "Very Clinical" about a patient in recovery on methadone who initially refused narcotics but later requested stronger pain medication after a procedure.
  • Alan's Perspective on Recovery and Pain: Alan shares personal anecdotes about his own recovery journey and experiences with pain management, including his thoughts on harm reduction and the importance of personal responsibility in recovery.
  • Ethical Considerations: The complexities of prescribing narcotics to a patient in recovery, the importance of open communication, and the value of consulting with other healthcare professionals (like the patient's pain management MD in this case).
  • Alternative Pain Management: Discussion of alternative pain management strategies, such as injectable or oral steroids (dexamethasone).

Alan is joined once again by the one and only Dr. Jason Smithson. We've often compared how dentistry is delivered differently between the United States and the UK, but today we compare how dental hygiene is delivered differently.

Summary: Dr. Alan Mead and Dr. Jason Smithson discuss the stark differences in how dental hygiene is approached in the United States versus the UK. They explore the reasons behind these differences, including insurance models, historical practices, and the perceived value of hygiene appointments by patients. The conversation also touches upon the role of diagnosis, consent, and duty of care in dental treatment.

Key Discussion Points:

  • Jason's Current Activities: Dr. Smithson discusses his teaching schedule at Spear Education, including new courses on aging teeth and other advanced restorative techniques. He also mentions his work in Australia and Europe.
  • The Hygienist Shortage: Alan describes his difficulty in finding a hygienist due to increased pay demands and a shrinking pool of candidates. This leads into the core discussion of the episode.
  • US vs. UK Hygiene Models:
    • US: Hygienists are often the primary point of contact for patients, with a focus on regular cleanings regardless of individual needs and disease. The system is largely insurance-driven. Patients often prioritize the cleaning over the dentist's exam.
    • UK: Dentists assess patients first, determining recall intervals based on risk factors (caries, periodontal disease, wear). Hygienist appointments are prescribed by the dentist and are therapy-focused (e.g., scaling and root planing for periodontal disease). Healthy patients may not see a hygienist at all.
  • Value Perception: Jason argues that the US system can make hygiene seem like a cosmetic procedure, diminishing its perceived value, especially for healthy patients. The UK model ties hygiene to specific therapeutic needs, increasing its perceived value.
  • Insurance and Payment Models: The discussion touches on the differences between NHS (government-funded) and private dental care in the UK, as well as the impact of insurance on treatment decisions in the US. The UK has a mixed system, while Dr. Smithson's practice is entirely fee-for-service.
  • Direct Access Hygiene (UK): In the UK, patients can directly access hygienists without a dentist's referral. This is uncommon in the US.
  • Diagnosis and Consent: Jason highlights the importance of explicit diagnosis before treatment, emphasizing that informed consent cannot be obtained without a diagnosis. He suggests that the US system sometimes relies on implicit diagnoses.
  • Duty of Care: Jason explains the concept of duty of care in the UK, stating that dentists assume responsibility for a patient's care once treatment begins. This raises questions about the US system and potential legal implications.
  • Efficiency and Multitasking: The conversation explores the inefficiency of jumping between patients and procedures, including hygiene checks during restorative work. Jason cites research on the negative impact of multitasking on quality and productivity.
  • Patient Expectations: The discussion acknowledges that US patients have been conditioned to expect regular cleanings, even when not clinically necessary. This creates a challenge for dentists who might want to shift to a more therapy-focused model.

Mohamed welcomes back Kevin and Zach to talk about the comprehensive exam. It turns out that they do them a little bit differently in dental school than in private practice! They discuss the challenges of lengthy school exams, the importance of efficient workflows in private practice, and the crucial role of patient communication.

Key Discussion Points:

  • Dental School Exams: Zach describes the lengthy and cumbersome process at his dental school, including screening appointments, multiple trips to radiology, extensive paperwork, and faculty approval. He highlights the frustration of the process and the difficulty of managing patient expectations and finances.
  • Private Practice Workflow (Kevin): Kevin details his streamlined approach in private practice, emphasizing the importance of delegation to trained team members. His workflow includes:
    • Comprehensive Records: High-quality photos (smile, retracted smile, occlusal, buccal), cone beam scans, bite wings, and potentially digital scans. He stresses the value of photos for patient education and documentation.
    • Efficient Exam: A quick but thorough exam after reviewing records.
    • Patient Communication: Using photos and plain language to explain diagnoses and treatment plans. Focusing on the patient's chief complaint.
    • Treatment Planning & Financial Discussion: Developing a plan and discussing finances with the patient.
  • Patient Triage: The hosts discuss the importance of phone triage to determine the type of appointment needed (emergency, comprehensive, etc.) and to schedule patients appropriately. They also touch on the challenge of "walk-in" emergencies and how to integrate them into the workflow.
  • Patient Communication: Both Kevin and Zach emphasize the use of patient-friendly language, avoiding technical jargon. Kevin highlights the importance of showing patients their photos and explaining findings visually.
  • Adapting to Patient Needs: The conversation covers tailoring exams and treatment plans to different patient populations (teens, elderly, etc.) and individual needs.
  • Teamwork: Kevin emphasizes the role of a well-trained team in managing the workflow efficiently.

Key Takeaways:

  • Photos are Crucial: High-quality photos are invaluable for patient education, documentation, and communication.
  • Streamline Your Workflow: Efficient processes save time and improve patient experience.
  • Communicate Clearly: Use patient-friendly language and visuals to explain diagnoses and treatment plans.
  • Triage Effectively: Proper phone triage helps schedule patients appropriately and manage the flow of the practice.
  • Adapt and Pivot: Be prepared to adjust your approach based on individual patient needs and emergencies.
  • Delegate Effectively: Trust your team members to handle tasks efficiently.

It today's episode, Alan has a conversation with Dr. Wade Pilling. Wade is a "big case" guy and a renowned expert in full arch dentistry. Dr. Pilling shares his insights on the importance of mentorship, the changing landscape of dentistry, and the ethical considerations surrounding full arch implant treatment. He also discusses his own journey in dentistry and offers advice to new dentists navigating the complexities of today's dental world. Tune in for a thought-provoking conversation about the future of dentistry and the importance of prioritizing patient care above all else.

Some links from the show:

Key Takeaways

  • The Importance of Mentorship: Dr. Pilling emphasizes the crucial role of mentorship in achieving clinical excellence in dentistry, especially for complex procedures like full arch implants. He believes that mentorship fosters a commitment to quality and patient care over financial incentives.
  • The Changing Landscape of Dentistry: Dr. Pilling discusses the challenges faced by new dentists due to high student debt and the increasing prevalence of DSOs. He expresses concern that the focus on money can lead to overtreatment and poor patient outcomes.
  • The Full Arch Challenge: Dr. Pilling cautions against new dentists jumping into full arch implant cases too early in their careers. He stresses the importance of proper diagnosis, treatment planning, and informed consent, especially when it comes to procedures that involve irreversible bone loss.
  • The Value of Experience and Continuing Education: Dr. Pilling shares his own journey of cobbling together training from various sources to become proficient in full arch dentistry. He highlights the need for ongoing learning and collaboration with experienced clinicians.
  • The Future of Dental Education: Dr. Pilling suggests a shift towards a more apprenticeship-based model for dental education, where students gain more clinical experience under the guidance of mentors.
  • Ethical Considerations: Dr. Pilling raises concerns about the marketing practices of some DSOs and the lack of accountability for patient outcomes. He advocates for a patient-centric approach to dentistry, where clinical excellence and informed consent are prioritized.

In this throwback episode Kevin and Zach discuss the importance of communication and managing patient expectations throughout the Invisalign process, from initial consultation to final results.

Key Takeaways:

  • Verbal skills are crucial: Effectively communicating with patients about the Invisalign process, including attachments, IPR ("slenderizing"), and potential refinements, is key to ensuring satisfaction.
  • Manage expectations early: Clearly explain the process, including potential discomfort, dietary changes, and the need for retainers, to avoid surprises and disappointment later.
  • Address objections proactively: Have answers prepared for common concerns like the appearance of attachments, the need for IPR, and the duration of treatment.
  • Use visual aids: Models, photos, or videos can help patients understand the process and visualize the expected outcomes.
  • Don't fight the "mostly satisfied": If an adult patient is happy with their results, even if not perfect, respect their decision and avoid pushing for further refinements.
  • Be prepared for the "never satisfied": Some patients may have unrealistic expectations or hyper-focus on minor imperfections. Patience, clear communication, and gentle guidance are essential.
  • Address noncompliance: Reiterate the importance of wearing aligners as prescribed and address non-compliance directly, but ultimately, patients are responsible for their own choices.
  • Invest in a good Invisalign manager: A dedicated team member can handle patient communication, manage expectations, and provide support throughout the process.
  • Remember the positive: While negative experiences tend to be more memorable, the majority of Invisalign patients are satisfied with their results.

In this episode of the Very Dental Student Podcast, Mo talks with Kevin and Zach to discuss endodontic procedures, comparing dental school protocols with real-world practices. They delve into a step-by-step walkthrough of an endo procedure on a premolar, highlighting key differences and offering valuable insights for dental students and new dentists.

Key Takeaways:

  • CBCT is Your Friend: While PAs and bitewings are helpful, a CBCT scan provides a 3D view of the canal system, acting as a "GPS" for navigation. This is especially crucial for molars and complex premolars.
  • Begin with the End in Mind: Always consider the final restoration (likely a crown). Take a pre-op impression before starting the endo to simplify temporary crown fabrication.
  • Quadrant Isolation: Isolate more than just the tooth being treated with your rubber dam. This provides better orientation and access for both endo and crown prep.
  • Flat Top that Tooth: Reduce the occlusion and flatten the tooth before accessing the canals. This improves visibility, access, and post-op comfort.
  • Lubricate Early and Often: Use RC Prep on your files before entering the canals to prevent pushing debris apically and to aid in lubrication and chelation.
  • Hypochlorite Timing: Introduce sodium hypochlorite after establishing working length to avoid pushing debris and potentially blocking the canal.
  • Secondary Rinses: Use EDTA or a similar product (like Smear Off) to remove the smear layer created by sodium hypochlorite before obturation.
  • Sealer Coating Technique: Coat the canal walls with sealer by running your files in reverse. This ensures a more thorough and even distribution.
  • Post-Op Comfort: Even if a crown is planned, taking the tooth out of occlusion can improve patient comfort and reduce post-op complications.
  • Posts - Controversial but Sometimes Necessary: While posts are less common today, they can be valuable in certain situations, particularly for premolars and anterior teeth.
  • Crown Prep Under Rubber Dam: While unconventional, prepping crowns under a rubber dam can be done effectively with practice and proper technique.

Buckle up, buttercup, because this is NOT your average episode of Very Clinical! Back in March of 2020, when Dr. Mac Jones was still one of the hosts of the podcast, they guys did a crossover episode with the Working Interferences crew (Dr. Josh Austin and Dr. Lance Timmerman) It was a wild ride of dental advice, shenanigans, and more than a few "colorful" words.

Highlights:

  • Special Crossover Episode: Very Clinical meets Working Interferences for the first time ever!
  • The Great Pacifier Debate: A Reddit question about "adult pacifiers" sends the guys down a hilarious rabbit hole of questionable online communities and unexpected discoveries.
  • Crown Prep Hacks: Even with the chaos, we manage to squeeze in some valuable clinical tips for faster and more efficient crown preps.
  • Nostalgia Overload: Relive the glory days of 90s chat rooms (ICQ, anyone?), 80s TV crossovers, and the one true "One Shining Moment" by David Barrett.

In this throwback episode, Kevin and Zach are joined by Dr. Nate Lawson to discuss the ins and outs of resin infiltration! They delve into the history of resin infiltration (specifically the Icon system by DMG), its clinical applications, predictability, limitations, and how to achieve optimal results. Dr. Lawson also shares insights from his own clinical experience and discusses his upcoming continuing education course on resin infiltration.

Key Takeaways:

  • What is Resin Infiltration? A minimally invasive technique used to arrest the progression of early caries and improve the appearance of white spot lesions (and sometimes fluorosis or other enamel defects). It involves etching the enamel, drying the area, and infiltrating it with a low-viscosity resin.
  • Advantages: Conservative, can halt caries progression, improves aesthetics, can be used on various types of enamel defects.
  • Disadvantages: Unpredictable outcomes, time-consuming procedure, can be expensive for single-tooth applications, results may not be visible radiographically.
  • Case Selection is Key: Ideal candidates include patients with white spot lesions following orthodontic treatment. Deeper lesions or those with significant staining may not respond as well.
  • Tips for Success: Multiple etching cycles can improve results. Managing patient expectations is crucial, as results may not always be perfect.
  • Long-term Results: Limited data exists on long-term outcomes beyond a few years, but the conservative nature of the treatment minimizes potential harm.
  • Continuing Education: Dr. Lawson offers a course on resin infiltration through UAB Continuing Education. The course focuses on case selection, clinical techniques, and achieving predictable outcomes.

Alan explores the concept of "automaticity" - the things we do without thinking. He discusses the pros and cons of this autopilot mode, drawing on examples from dentistry, daily commutes, and social media habits. He encourages listeners to be mindful of their automatic behaviors and make conscious choices to break unhealthy patterns.

Key Takeaways:

  • Efficiency vs. Mindlessness: Automaticity can be efficient, like in established dental routines, but also lead to mindless, unhealthy habits like emotional eating or excessive social media use.
  • Intentionality is Key: Be aware of your automatic actions and assess if they're serving you well.
  • Breaking Bad Habits: Consciously interrupt automatic behaviors and replace them with healthier choices. Tools like website blockers or the "Brick" device can help.
  • Social Media Habits: Alan challenges listeners to examine their social media use and consider strategies to break free from automatic scrolling.

Alan interviews Dr. Thom Caspers, a former Seattle-area dentist who now teaches at Midwestern University College of Dental Medicine in Arizona. They discuss the early days of Dentaltown, the transition from private practice to academia, and the unique aspects of Midwestern's dental program, including its focus on the private practice model and comprehensive patient care. They also delve into the challenges and opportunities facing dental students today, including the impact of social media, rising tuition costs, and the evolving landscape of dental technology.

Key Takeaways:

  • Dentaltown's Influence: Dentaltown played a crucial role in shaping Thom and Alan's careers and connecting them with other dentists.
  • Midwestern's Unique Model: Midwestern's dental program stands out for its private practice model, where students work in suites with experienced faculty, providing comprehensive care to patients. This approach fosters continuity of care, mentorship, and real-world experience.
  • Competency-Based Education: Midwestern emphasizes competency-based education. This contrasts with traditional dental schools that often rely on a set number of procedures.
  • Financial Realities: Dental students today face significant financial burdens due to rising tuition costs. While Midwestern offers some opportunities for loan forgiveness, many students are drawn to DSOs for their financial incentives.
  • Social Media's Impact: Social media presents both challenges and opportunities for dental students. While it can provide valuable information and connections, it can also contribute to anxiety and unrealistic expectations.
  • Technology in Dental Education: Midwestern embraces technology, offering students access to CAD/CAM systems for same-day dentistry, but significantly less emphasis on traditional lab work.
  • The Changing Student: Thom observes that today's dental students are different from previous generations, with higher levels of anxiety and a preference for technology-driven learning.

Alan welcomes frequent guest and co-host Dr. Matt Standridge back to Very Dental. They dive into the evolving landscape of dental practice ownership, comparing traditional solo practices with the increasing prevalence of DSOs. The conversation explores the current incentives and hurdles for new dentists, the impact of rising dental school debt, and the practical aspects of introducing advanced procedures like aligner therapy and all-on-X implants into general practices.

They also touch on the phenomenon of influencer culture in dentistry and its implications. Mostly they just laugh about our absurd profession and how it continues to change.

Key Takeaways:

  • Private practice ownership is becoming increasingly challenging due to factors like rising costs, insurance reimbursements, and patient expectations.
  • The "one doctor, one owner" model may become less common, with more practices incorporating associates or transitioning to alternative models.
  • Many young dentists are less driven by ownership and more interested in alternative career paths, including becoming a dental influencer.
  • Overuse of all-on-X procedures is a concern, with potential for complications and ethical considerations.
  • Orthodontic training, particularly with aligners, offers a high ROI and can improve overall dental skills.
  • "Bread and butter" dentistry still has value and shouldn't be dismissed.
  • Beware of "style over substance" in dentistry, particularly when it comes to advanced procedures.
  • Mentorship (or "coaching") can be valuable, but find trusted sources.

Alan is joined by the one and only Dr. Brian Baliwas. You might know him better as @sfdentalnerd on Instagram, where he is probably the most entertaining dental follow on the platform!

Alan and Brian have a lively discussion about the frustrations of dental insurance, the perils of planned obsolescence in dental equipment, and the importance of transparency in dentistry. They delve into the surprisingly efficient insurance model used for Dr. Baliwas's dog, Xylo and contrast it with the often opaque and frustrating world of human dental insurance.

The conversation also touches on the challenges of running a fee-for-service practice, the hidden costs of dental procedures, and the value of patient communication.

Key Takeaways:

  • Dental insurance vs. pet insurance: Dr. Baliwas highlights the stark differences between the transparent and efficient claims process for his dog's insurance and the confusing world of human dental insurance. This raises questions about the role of employers, transparency, and the overall value of dental insurance plans.
  • Planned obsolescence: The conversation explores the frustrating trend of planned obsolescence in dental equipment, with examples like air polishers and intraoral scanners. Dr. Baliwas shares his experience of repairing his own equipment due to a lack of available parts and discusses the push towards subscription models for dental technology.
  • Fee-for-service dentistry: Brian discusses the benefits and challenges of running a fee-for-service practice, emphasizing the importance of accurate cost analysis and transparent patient communication.
  • The value of hygiene: Brian and Alan discuss patient perceptions of dental hygiene and the importance of comprehensive exams in a preventive dental model. They also touch on the use of air polishers and guided biofilm therapy.
  • The importance of company support: When choosing dental products and equipment Brian stresses the importance of excellent company support and customer service rather than simply focusing on the "best" brand names.

Alan welcomes back Dr. Melissa Seibert to talk about caries removal. While academics and researchers don't find the idea of incomplete caries removal controversial, many clinicians just can't get their head around it! Melissa drops a LOT of knowledge on us in this episode!

Key takeaways:

  • Selective caries removal is a well-substantiated approach to caries management that involves removing only the infected dentin while leaving affected dentin behind. This can help to preserve healthy tooth structure and avoid pulpal exposure.
  • Magnification and light are essential for accurate caries diagnosis. Melissa emphasizes the importance of "sharp eyes and a dull explorer."
  • Radiographs are crucial for monitoring caries progression and making informed decisions about treatment.
  • SDF can be a valuable tool for arresting caries in both children and adults.
  • Sealants, especially glass ionomer sealants, can be effective in preventing caries, but proper isolation is essential.
  • The sandwich technique is an older technique that is not as commonly used today.
  • Vital pulp therapy is a topic that will be explored in a future episode.

Alan welcomes Ruben Kamp, the founder and a ransomware expert at Darkhorse Tech.

Dental practices are very susceptible to ransomware. We all like to think that it won't happen to us, but between 7 and 9% of dental offices suffer ransomware attacks every year! Are you taking all the precautions that you can?

Reuben and Alan discuss the critical issues of ransomware attacks and cybersecurity in dental practices. They explore real-life incidents, including how ransomware infiltrates dental offices (spoiler: Reuben says it's about 91% through email)

They also talk about the severe implications such attacks can have, from damage to your reputation to legal liabilities. Ruben provides comprehensive advice on preventive measures, including the importance of firewalls, regular updates, team training, and cybersecurity insurance.

They emphasize that while ransomware threats are pervasive, proper security protocols can significantly mitigate risks. The session also touches on the intricacies of backups, cloud-based software, and the evolving nature of cyber threats with advancements in AI.

  • David is REALLY glad to be back to in person teaching!
  • David explains how he became such a lab and materials focused clinician
  • "Knowing exactly what they're doing in the lab has made me a better dentist."

  • Do you know who is creating your restorations?
  • Your lab techs know what kind of dentist you are!
  • Let's go to Lab Day in Chicago!
  • Do you do CE with your lab tech(s)?

  • Bridges are legitimate treatment in a lot of cases and implant are not always the best choice.
  • The greatest trick for strengthening a zirconia bridge!
  • Becoming David Hornbrook
  • Photography vs. "Instagram Photography"
  • Patient expectations and "ideal" dentistry
  • Weighing minimally invasive techniques against creating room for materials
  • Informed consent and how the dentist "spins" the treatment
  • David's recommendations for new dentists:
    • find a mentor
    • eliminate the undesirable procedures from your practice as soon as possible
    • realize that you're going to be a dentist for a long time
    • "hurry up and slow down"
    • Be uncompromising

Al is joined once again by Dr. Dawn Grimes. She was on a couple months ago and we didn't get to some of the stuff that Al wanted to talk about...so we're here for round 2!

One of these things is the lie that we tell ourselves about time. See if this one seems familiar to you:

"Once x is done, I will have more time for y."

And no matter what, once you free up some time, other things will grow to fill that time.

Some of what we discussed:

  • Evaluating wasted/sunk time
  • Why the Matrix movies are not action movies at all
    • "There are days that I just want the steak."
  • We don't want reality and we often won't see reality
  • Instagram and overtreatment
  • You're going to have to replace things over time
  • Root canals don't cause cancer
  • "Blushing pulp"
  • "That tooth didn't hurt until you worked on it."
  • Better case acceptance = don't care if they do the treatment
  • Play hard to get
  • You cannot care more than the patient does
  • Patients aren't doing care for their dentist. Don't work on patients that are "doing it for you."
  • Diagnose and explain like Spock...emotionless

Social media is a blessing and a curse. Yes, it's self administered poison and yes, it can be bad for your mental health.

So...you should quit. Or never start.

But...if you choose to use it, you should curate what you see. Al gives some tips on this.

Imagine being a patient who shows up for their appointment only to be turned away because the office's server was down. Now imagine if you're a software guy and this happens to you! That's the story with John Hammond, co-founder of Neem, a cloud based practice management software!

Alan and John have a wide ranging discussion about the advantages of cloud based practice management!

  • It's not the technology as much as the switching
  • The Neem elevator pitch
  • The pros and cons of cloud computing in dentistry
  • Why has medicine jumped into cloud so easily?
  • The key to Neem...painless migration
  • Two options for imaging with Neem
  • Making Neem intuitive
  • Dental software is less sexy than a lot of other technology
  • Intelligent forms that patient input built in
  • Real time insurance eligibility
  • Will cloud be in the majority in 10-15 years? (software has a shelf life)
  • DSOs and the cloud

This throwback episode features how we introduced the Very Dental Student Podcast! Mohamed talks with Al for a couple minutes and then brings in Dr. Brandon Evert to talk to him about the transition from student to dentist!

Original show notes:

Welcome to the inaugural episode of the Very Dental Student Podcast! Alan introduces your host, Mohamed Abo-Basha, who is a D2 at University of Oklahoma! Al's tips for dental students:

  • If you can swing it, many dental companies will sell stuff to dental students at a pretty great discount (think loupes, headlamps, etc.)
  • Take advantage of your dental student email address!

Then, Mohamed brings the one and only Dr. Brandon Evert on for an amazing interview!

  • Brandon's take on dental school
  • Learn what kind of student you are (morning person, night person) and learn how to organize a routine
  • Comparing and contrasting dental school styles (case studies vs. route memorization)
  • When did you first get patient contact?
  • Brandon was heavy on reps in the sim lab (note from Al: that makes Brandon smart!)
  • Get used to indirect vision in preclinic!
  • Brandon's first year out and his 4 year military commitment
  • Military = seeing LOTS of dentistry
  • Rotations at Paris Island (CAD/CAM, endo with a scope, surgery)
  • Is the military experience worth it?
  • Brand explains the financial aspects of a military scholarship ("it's not just a way to get free dental school.") and the non-financial aspects
  • HPSP vs. HSCP
  • Brandon's long term plan for private practice

"They always talk about how the independent dentists are providing better care than the groups are, the corporate dentistry. Well, that's not necessarily true. If you really study the tools that they have that the independent dentists have not yet adopted, most of the independent dentists are far behind the invisible DSOs and DSOs from a patient care and a technology adoption."

Chip Fichtner, Large Practice Sales

Alan is joined once again by Chip Fichtner of Large Practice Sales to talk about Invisible DSOs. Some of our topics included:

  • How did Chip get in to IDSOs?
  • Large Practice Sales only represents doctors!
  • Why is dentistry a focus for investors?
  • Consolidation in mortuary science, veterinary medicine and medicine and how they differ from dentistry
  • The "Walmart-ization" of dentistry
  • Technology and AI in IDSOs.
  • Chip looks into his crystal ball
  • Who will buy an average practice (not a large practice)?
  • IDSO = full autonomy for owner dentists
  • IDSOs and specialists...an interesting fit
  • What does a "bad" experience with an IDSO look like?
  • What does a "bad deal structure" in an IDSO transaction look like?
  • How was COVID good for us?

Does an invisible DSO sound like something that would be a good fit for you?  Get in touch with Chip at Large Practice Sales!

Alan is joined with Dr. Armen Mirzayan and Damien Bonner of CAD-Ray for a second round of discussion on digital dentistry!

  • Damien explains a typical progression of someone moving into digital dentistry.
  • Companies shouldn't sell digital products to dentists that aren't ready for them.
  • Armen is a very cheap salesperson.
  • Buy your scanner from someone who knows something about scanners.
  • CAD-Ray are terrible sales people but amazing support people.
  • Armen on scan bodies
  • One very cool thing about digital dentistry is that we can verify accuracy in ways that you can't in analog dentistry.
  • Digital margins are tiny enough that we cannot manufacture to the tolerances that we actually see on the digital impression.
  • The slop is in manufacturing more than digital scanning
  • Photogrammetry and the accuracy of digital scanners over an edentulous scan.
  • How to make an accurate scan for full arch implants
  • Intraoral photgrammetry with the Shining 3D Aoralscan Elite
  • This is going to start making smaller implant cases faster and cheaper!
  • Why start a teaching center in Vegas?

Al riffs on a stressful patient problem/story that occupied his brain for an entire day and how quickly it was forgotten for the next drama.

It's worth remembering that the drama that's occupying your mind right now is something you probably won't even remember in a week.

Melissa has taken a TON of CE in order to learn how to do AS MUCH AS POSSIBLE as quickly as possible. She thinks that there was a lot of pressure to be a super dentist early in her career. She did this and it was disruptive to her family life and can be a financial strain. The big message...slow your roll.

Al is joined by 3rd year dental student and host of the Very Dental Student podcast, Mohamed Abo-Basha!

Their conversation focused on what it looks like to be a 3rd year dental student at the University of Oklahoma College of Dentistry. Al liberally sprinkles stories of mid-90s University of Minnesota dental school as well. If you've graduated you'll enjoy the nostalgia (and that you're done!) and if you're a dental student you'll commiserate!

As a 3rd year dental student, Mohammed wonders if the protocols that he's learning in dental school are applicable to real life. He sat down to interview Alan Mead about "real life dentistry."

  • Bonding protocols in real life
  • Etching vs. self etch
  • Alcohol vs. acetone based bonding agents
  • Composite wedging and matrices
  • Heated composite and "snowplow"
  • Indirect workflow and lab communication
  • Diagnosing cracked teeth/cracked tooth syndrome
  • How do you know when a tooth is unrestorable?

It turns out that Lance isn't fond of a giant, hygiene-centric practice. After COVID he found that dental hygiene barely covered its own overhead and he definitely didn't enjoy it.

Al riff's on a listener's suggestion. The listener suggested that specialists seem to get very gentle treatment and reviews on social media platforms, but somehow whenever general dentists or pediatric dentists come up, people seem to lose their minds. Why is the image of generalists so much worse than specialists?

What does it mean when a patient tells you that "I found you from the internet?" Well, practically nothing. Did they see your website? Did they read a review? Did they see a social media post? Unless your front desk is REALLY good at figuring this stuff out, you may not be able to tell where your marketing is really working and where it might be missing.

Michael Anderson and Laura Maly of the Wonderist Agency join Alan to talk about this problem and a solution that they will be rolling out. What if you could automatically connect your marketing result and your practice management software to show exactly where your marketing is working and the dollars and cents it is creating in your practice!

Social media tells the story of influencers that are killing it while living their best lives. But here in the real world, some people are living in crises. There are people suffering with mental health issues all around us and it's not always obvious who is.

Al received a couple of pretty scary anonymous letters that tell a story of mental illness and anger at the dental industry that's worth thinking about.

Alan welcomes back Dr. Mike Melkers for a wide ranging conversation. Mike and Al go WAY back (DentalTown in the "oughts" anyone?) and we catch up a little bit.

Al was affected by a video Mike recorded earlier this year contrasting "comprehensive" care vs. "appropriate" care.

What is comprehensive care? What is appropriate care? Mike is the guy to help us understand appropriate care for the dentist and the patient.

  • Meet the patient where they are, for what their goals are
  • The shame of not doing all FMR's
  • Do patients have "goals?"
  • Values vs. ethics
  • Think about the "why?"
  • Considerations not consequences
  • "What are your expectations for your teeth at x" (insert an age)
  • "Are you aware that you're missing a tooth here..."
  • Purpose, Process and Presentation

Today is a throwback episode featuring Kevin, Zach and Al talking about how they've evolved on caries removal. It's a classic and I hope you enjoy it!

Original show notes:

Dr. Alan Mead of the DentalHacks podcast was gracious enough to join us again this week.

And we start off with old school internet videos:

Bonus:   He got a web redemption!

Then we got into some hot caries removal talk.

  • What burs do you use?
  • To dye, or not to dye?
  • Do you "leave" any caries? Why or why not?

Caries removal is a divisive topic. Did we get it right? Wrong? Let us know in the Very Clinical Facebook group!

Today's throwback episode features the one and only Dr. Rob Ritter. Al talked to Rob back in late 2021 after having a couple of bad clinical beats and Rob has some pretty good advice.

Original show notes:

Alan is joined by Dr. Rob Ritter to talk about a couple of bad beats he had. Perfing an endo and punting a tough extraction had Al wondering if he should be doing these procedures in his office. Rob is a proud “restorative dentist” by choice and has described his approach as “from the gums up.” How did he decide what he wanted to include in his practice? Why did he decide to leave a lot of “specialty” procedures out? Some topics we covered:

  • Rob’s course, “The Protocol”, the dates for 2022 are available and registration opens on September 14th. If you’re interested, you better sign up fast because they’ll sell out immediately! (limited to 14 people!)
  • No compromise treatment
  • “Just because you don’t want to do the right thing doesn’t mean I’m going to do the wrong thing.”
  • “When you start feeling bad for patients, you start making bad decisions”
  • Late 90’s “needs based” dentistry to “wants based” dentistry
  • Doing dentistry because you NEED to (financial constraints) is more common
  • Al’s bad beats and walking away from endo and surgery (and why it didn’t last)
  • Case selection...don’t let your guard down!
  • You don’t HAVE to do anything you don’t want to
  • No shame in referring anything that you don’t want to do
  • Find a referral network that is an extension of your practice
  • Rob ALMOST took a tooth out earlier this year...and then decided against it. It was a wise choice.

Today's throwback episode features a discussion on when to throw in the towel. Keep restoring or moving to a different kind of plan is a big deal these days. The guys have thoughts.

Also...the baseball talk is 3 years out of date. FYI.

Previous show notes:

We open with a full 11 minutes on baseball so if you just want the dental chat, start the podcast a few minutes in.

This week we have a short chat on terminal dentition.

What is terminal dentition?  How hard is it to make that call?

Is it harder to make that call when you’ve seen the patient for 10+ years then a new patient?

When do you quit restoring?

What clinical criteria vs. patients health vs. financial considerations go into the decision?

How is your comfort level with dentures, extractions, pre-prosthetic surgery, etc?  Does that factor into your personal choice of whether to recommend dentures vs restorations.

Please join the Very Clinical Facebook group to discuss this episode and much much more!

Mohammed is joined by Dr. Brandon Evert and Dr. Seth Braker! Brandon and Seth were classmates throughout dental school and have contrasting early dental career paths! Brandon is a military dentist and Seth went directly into private practice.

Brandon started the "Extracting Wisdom" podcast before he started dental school and later asked Seth to come on as a co-host where they contrast their experiences!

This throwback episode features a replay of a Dental Hacks episode featuring Brian Colao of Dykema!

Brian Colao knows DSOs. He's an attorney that helps people create and administer DSOs. He's been a frequent guest on the Dental Hacks and he gave Jason and Alan some of his time at the recent Voices of Dentistry meeting. We went deep on the topic of DSOs and covered a lot of information:

  • What is a DSO? (spoiler: "A DSO is an entity that provides nonclinical administrative support services to a dental practice that is owned by a licensed dentist.")
  • From one practice to thousands
  • When did we start to really see DSOs take hold
  • The episode where we met Brian, "DSOs and Digital Smile Design (DHP186)"
  • Was there a DSO bubble?
  •  Denovo vs. Acquisitions
  • What to do if a DSO approaches you?
  • How do you research a DSO that might want to acquire your office?
  • Brian thinks that there is probably a DSO that could fit most any office...go to a conference
  • What are DSO deals looking like for selling dentists?
  • The facts from Brian's mouth to your ears: DSOs are at 23% market penetration in the U.S. dental market right now. He predicts that there will be 80% market penetration in 7-10 years. He feels that you should plan accordingly.
  • EBITA: "Earnings Before Interest, Taxes, and Amortization"
  • What does a DSO look like?

Al riffs on the concept of "price anchoring." If you can plant a dollar amount in the brain of your customer, you can manipulate what they believe a product or service is worth. It works with iPads as well as dental care and insurers are way better a this stuff than we are.

Alan is joined by return guest Dr. Gary Holtzclaw to talk about different models of dental practice. Gary and Al have a Facebook chat that's been going for awhile about a lot of different models of practice that they've thought about and they hash some of them out on today's podcast! Some of the highlights:

  • Where did the "hygiene driven" model come from?
  • Can an emergency only model work?
  • "Wants-based" models vs. "needs based" models
  • The landscape for new dentists in 2024 vs. 20+ years ago
  • Who can pull off a startup?
  • Is assisted hygiene the future?
  • Will "biofilm management" become the new "hygiene?"
  • Why can medicine get away with such poor service?
  • Are financial conversations required for informed consent? If yes, how does that work in medicine?
  • Do orthopedic surgeons talk to their colleagues about different models of orthopedic surgery delivery?

And so much more!

Dr. Randy Houska joins Kevin and Zach to talk about things we've changed our minds about.

Today's episode continues the series for Zach and Kevin where they talk to some experienced dentists about what things they've changed their mind about over the years. Today's episode features a discussion with Randy about life in general!

  • Randy has stopped worrying about taking time off. You can't get that time back.
  • Take more time off!
